**Position Summary**:
The Williams Lake First Nations Wellness Centre (WLFNWC) is a new primary care centre opening to improve access to culturally safe care for First Nations people and their families.

Multiple health professionals will work together to provide team-based care based on local First Nations traditional ways of knowing, being and wellness.

The Wellness Navigator will work with individuals and families to advance their health and wellness by supporting them as they access the system. They will also provide wellness planning and increase health literacy.

**Responsibilities**:

- Models cultural safety, humility, relational practice and a trauma-informed approach;
- Leads the mapping of the local, regional and provincial health and social systems that are important for improving the health and wellness outcomes for First Nations people, including those living with chronic conditions;
- Works with individuals and families to advance access to health and wellness services, and empowering health and wellness by championing the concept of developing individual health and wellness planning;
- Supports the development of health literacy work plan including providing tools and support for each client to have an individual health and wellness plan;
- Develops, recommends and implements the Centre’s practices and procedures to improve access to health care and a wide array of health and wellness programs, services and facilities;
- Supports the facilitation of a person-centered and holistic approach to health and wellness journeys to improve the healthcare experience of Indigenous patients within the hospital system, including anti-racism, cultural safety, family support, advocacy, spiritual care, community liaison, etc.;
- Develops partnerships with community health and wellness service providers and local health care providers in offering spiritual, mental, emotional and physical wellness of community members, and improve transitions in care and access to appropriate primary health care and public health services;

**Qualifications**:

- You are passionate about supporting First Nations individuals and families on their health and wellness journeys;
- You have a relevant Bachelor’s degree and 5-7 years’ experience supporting the coordination of client care;
- You have experience in the development of health and wellness plans in collaboration with the client, their family, and the multi-disciplinary primary care team;
- You have experience working with those living with chronic conditions; as well as in-depth knowledge of Aboriginal/First Nations health and wellness system needs, and a comprehensive understanding of community, public, and primary health care systems.
