Reporting to the Director Outpatient Care Transformation and Clinical Space Lead, the Manager, Outpatient Care Transformation will play a key role in supporting the transformation of outpatient care at THP. The Manager will work collaboratively with senior leaders, patient care services management, and other stakeholders to develop and implement initiatives that support the delivery of consistent, high-quality, patient-centred care to our outpatient population. This work will support, inform and advance Trillium Health Partners' Master plan including Trillium HealthWorks project design of clinical programs and services across the outpatient care platform.

**Key Responsibilities**:

- Program Development and Implementation_
- Support the development and implementation of Outpatient Care Transformation initiatives, including clinical practice standardization, Epic workflow related to centralized access and intake, technology, and space planning.
- Ensure that Outpatient Care Transformation initiatives are evidence-based, supported by data and analytics, and aligned with best practices and the THP strategic plan.
- Work with clinical leaders to identify areas for improvement in Outpatient Care and develop strategies to address these areas.
- Key liaison between Trillium HealthWorks team and Outpatient program leadership, ensuring alignment with operational planning and approved scope in Trillium HealthWorks projects
- Clinical Program and Space planning_
- Participate in the corporate and PSC priorities on related to program planning that may have a space impact, informed by the annual work plans.
- Responsible to work with programs to identify downstream impacts of any program planning and associated space decisions.
- Responsible for THP’s broader outpatient patient care strategy and key contact for coordination of Trillium HealthWorks outpatient care planning projects
- In partnership with Capital Planning and Redevelopment, ensuring that space planning standards, and associated principles, set through Trillium HealthWorks project are being integrated into corporate space planning activities.
- Project Management and Execution_
- Work collaboratively with cross-functional teams to execute Outpatient Care Transformation initiatives.
- Collaborate with Patient Care Services leaders to ensure that Outpatient Care Transformation initiatives are integrated within other programs and services across THP, and with the Trillium HealthWorks projects.
- Lead/participate in the development and delivery of presentations, reports, and proposals
- Develop and administer project management framework, processes and tools - including the use of portfolio management and project management tools and support a community of practice for project management
- Performance Monitoring and Reporting_
- Monitor and evaluate corporate performance metrics related to progress on Outpatient Care Transformation initiatives.
- Provide regular reports on progress to the Director, Outpatient Care Transformation and other stakeholders.
- Development of an outpatient care dashboard
- Change Management and HealthWorks Operational Readiness Planning_
- Work with the Director, Outpatient Care Transformation to facilitate change management for clinical practice standardization.
- Support Trillium HealthWorks teams as work transitions from design into Operational Readiness planning to ensure alignment across programs
- In collaboration with the Trillium HealthWorks team, support Operational Readiness within the impacted outpatient programs through a series of change management planning anchored on the guided principles within the quadruple aim of - improved clinician experience, improved patient experience, better outcomes and lower costs.
- Ensure that staff are well-informed and engaged in achieving the vision and goals of THP.
