**Job Description**:
**Reporting to**:Director Patient Experience, Quality, Risk, Privacy & IPAC
The Patient Safety Lead advocates for excellence in patient safety by developing and implementing structures, and supporting best practies to enhance patient safety. He/she will foster a Just Culutre by promoting the value of incident reporting, conducting incident reviews, tracking implementation of recomemndations, and sharing systemic learnings. He/she will be the system administrator for Report Link (RL) incident reporitng system, and will be the Accreditation Canada Lead. He/she will support the annual Quality Improvement Plan (QIP) cycle.
**Accountabilities**:
- Accreditation Lead - support Director Patient Experience, Quality, Risk, Privacy & IPAC as the organizational lead for all preparation activities. Assemble evidence for submission, as required. During non-survey years continual review of new/emerging standards.
- RL System Administrator - develop and refine the incident capture screens; development of reports and interpretation of unit level and corporate results; review and close all RL events.
- Just Culture - promote through refinement of policies, practices and sharing of systemic learnings
- Patient Partner - promote inclusion of patient/care partner voice in patient safety projects and improvement work including annual QIP development
- Incident Reviews - develop and implement policies supporting; tracking of recommendation implementation; support staff as they participate in incident reviews
- Quality Improvement - using data and Quality Improvement tools lead multidisciplinary project teams to implement various patient safety projects. Initiatives could be aligned to emerging Accreditation Canada standards, annual QIP metrics, identified organizational risks, and/or legislative changes.
- Works with leadership to develop strategic linkages with agencies active in patient safety (i.e. IHI, CPSI, HQO, OHA, MOHLTC).
- Works closely with teams to develop, implement and evaluate the annual Quality Improvement Plan (QIP)
- Collaborates on the development & implementation of a multi-year Quality & Patient Safety Plan including tactics, success metrics, and regular prioritization and re-prioritization of initiatives to achieve the overarching measure(s) of success
**Qualifications**:
- Bachelor Degree in a related healthcare field (e.g. BScN, BPharm etc.) required
- Master’s Degree (or in progress) in a related field an asset
- Member of a Regulated Health Professional Association college required
- Minimum of five (5) years progressive clinical experience required
- Formal Patient Safety training, experience, and/or designation required
- Proficiency with RL incident reporting system required
- Proficient in Meditech or alternative Hospital Information System required
- Familiarity with Accreditation Canada Required Organizational Practices, Institute for Safe Medication Practices an asset
- Proficient in Microsoft Office suite products (Word, Excel, Power Point, Visio) required
- Demonstrated exceptional interpersonal, verbal and written communication skills including the ability to foster collaborative relationships is required
- Demonstrated ability to comfortably present similar material to varied interest groups (e.g. Board/MAC to front line staff) is required
- Demonstrated excellent project management skills required - including the ability to co-manage multiple projects simultaneously
- Experience in Quality Improvement methodology (e.g. Lean, IHI Model for Improvement or equivalent) is required
- Experience in aspects/theory of change management is an asset
- Experience with Design Thinking and/or Experience Based Design methodology an asset
- Strong organizational skills are required
- Working knowledge of relevant legislation - Excellent Care for all Act, Quality of Care Information Protection Act, Public Hospitals Act, Apology Act
