Community Development Worker

**Position Summary**

As part of a multi-disciplinary team, the community development worker will advocate for community resources and assist complex clients in connecting with needed supports and services, particularly those who are at risk of, infected with and affected by Hepatitis C or HIV. The Community Development Worker works with community groups, organizations, and municipal partners to support and develop community initiatives, client engagement tools, and programs to support marginalized populations. The Community Development Worker will address factors which affect community and individual health through client advocacy and case management as well as promotion of health programs both at SHC and in the community. The CDW will provide outreach to marginalized community members living with/at risk of acquiring hepatitis C and HIV. This position will provide information and support services and promote referrals to health and social services.

Community Development
- Work with community partners to identify and assist with the development of various health promotion initiatives, projects and programs that serve the target population of Street Health Centre
- provide outreach and information sharing to marginalized community members
- Identify community and systemic issues facing marginalized populations
- Promote awareness of and participation in Street Health Centre and KCHC activities and programs Coordinate volunteer opportunities and build capacity within the peer program
- Provide mentorship, guidance and skills-building to the peer program
- Maintain a current knowledge of community resources pertinent to the particular client base

Client support and system navigation
- Provide direct service to clients; assist complex clients facing barriers to care with system navigation and improve access to supports
- Assist clients with tax supports and accessing related resources
- Advocate for clients and assist with connecting the client to community resources and supports to help address unmet social health needs (e.g. housing, income security, food security, transportation, literacy, and social isolation)
- Make internal and external referrals, connecting clients to organization and community resources

Administration
- Routinely compile, enter and report confidential data per SHC and funder’s request;
- Perform administrative tasks as related to the position, including activities such as time planning, documentation, and report preparation;
- Record client encounter data and complete reporting requirements for funders
- Ensure that all necessary reports are completed and submitted to the appropriate sources meeting set deadlines
- Work within the agreed upon budget for each program/initiative.

- Supports KCHC’s student and volunteer placement programs
- Promotes awareness of and participation in KCHC activities

- On occasion, perform other temporary duties as required

**Basic Education and Experience Requirements**
- Undergraduate degree in relevant discipline from a recognized university or combination of some post-secondary education
- Minimum three years’ experience in direct client support
- Thorough knowledge of and experience with issues relevant to clients with Hepatitis C and HIV (such as, but not limited to harm reduction, addictions and mental health);
**Knowledge, Skills and Abilities**
- Knowledge of harm reduction programs, health promotion initiatives and social determinants of health perspective.
- Knowledge of social service benefits like OAS, ODSP and filling Canadian taxes.
