Reporting directly to the Manager, the incumbent will provide analytical and data management support for the Ontario Renal Reporting System and high level hemodialysis patient flow and coordination for the assigned area of the program.

The incumbent will also review, analyze, and interpret utilization results and prepare reports and statistics. Liaises with internal and external providers, patients/families for the purpose of patient flow and utilization.

**Duties and Responsibilities**:

- Collaborates and consults with multiple stakeholders to prioritize patient flow both internally and externally.
- Collaborates with the patient care team(s) to support patient flow.
- Using excellent decision making and problem solving skills to prioritize and resolve the day to day patient flow issues.
- Accountable for assigning patient to a treatment roster and notifying care team. Ensures patients are placed in the most appropriate hemodialysis treatment spot.
- Advises stakeholders of overall hemodialysis bed/chair status on a daily basis. This includes admissions, discharges and transfers.
- Maintains and tracks daily statistics - admission, discharge, transfer and travelling patient lists, as required to facilitate patient flow.
- Collaborates with patients/families to manage requests in a timely manner based on priority.
- Manages and coordinates patient travel - liaises with patients/families in regards to travel plans, documents required, etc.
- Liaises with patients/external partners when patients are travelling to SMH.
- Leads and/or participates in quality improvement initiatives as it relates to patient access and flow for hemodialysis patients.
- Ontario Renal Reporting System (ORRS) patient level data entry.
- Manages data validation activities to endure data is complete, accurate and of high quality.
- Manages audits and data cleansing activities related to ORRS.
- Reports and investigates data quality concerns and reconciles data errors.
- Develops data reporting tools.
- Reporting and distribution of data to key stakeholders.
- Removes barrier related to data collection and validation.
- Analysis and forecasting of data from a patient flow perspective.
- Maintains documents that are required for reporting.
- Manages and maintains primary Nephrologists’ patient assignments.
- Leads and/or participates in local quality assurance initiatives as it relates to ORRS data management.
- Monitors and reports all ORRS activity that may have a financial impact on the program.
- Preparation of data analysis and presentations of critical renal indicators to recommend areas of financial concern

**Qualifications**:

- Community College Diploma, undergraduate Degree in Science preferred
- Certificate in project management or working towards with a completion required
- A minimum 5 years current hospital experience in a fast paced environment.
- Demonstrated knowledge of the Ontario Renal Network and Ontario Renal Reporting System (ORRS) with at least 5 years of ORRS data entry experience.
- Demonstrated ability and accuracy with data entry into ORRS.
- Demonstrated quality assurance initiatives as it relates to ORRS data management.
- Demonstrated strong analytical, conceptual and problem solving skills.
- Demonstrated ability to use critical thinking skills in the review and analysis of metrics and financial data.
- Demonstrated ability to perform data validation activities to endure data is complete, accurate and of high quality.
- Excellent interpersonal and communication skills.
- Excellent computer skills e.g., Microsoft Word, Excel and Powerpoint with proven experience in data manipulation, interpretation and extraction of information.
- Ability to multi task while producing high quality work required;
- Ability to prioritize and identify relevant support for the resolution issues in the day to day operations related to patients flow and data management.
- Industrious, organized, methodical worker with ability to use initiative, good judgment and pays attention to detail.
- Excellent customer service, interpersonal and listening skills with the ability to effectively interact with staff all levels required;
- Ability to take initiative and work in a self-directed manner as required;
- Demonstrated team player with the ability to be flexible and adaptable to change required;
- Demonstrated commitment to and understanding of the Hospitals Mission and values
