The position will:

- Provide timely response to referrals.
- Provide intensive case management support, transition, and stabilization for at risk seniors living with dementia and complex needs and/or care partners through coordinated care integration and collaboration with health and community support services and members of the care team.
- Be flexible to increase intensity and frequency of direct support according to the needs of at-risk seniors living with dementia and/or care partners.
- Work with existing community programs and services, local hospitals, Home and Community Care Support Services, primary care to provide “wrap around” support and services.
- In-home holistic assessment
- Development of behavioural care plans utilizing non-pharmacological techniques
- Service coordination
- Education and coaching
- Person-centered care planning
- Overall system navigation.

This is an outreach position that works in the community (in clients' homes, community spaces, etc.) and requires travelling, and occasional work during the evening and the weekend. The hourly range for this job is $23.74 - $35.60 ($43,200 to $64,800 based on 1820 hours annually). We also offer a competitive Group Benefits and RRSP package.

This position is located in our Hamilton office and includes travel within the Hamilton region. Office hours are Monday to Friday from 8:30am to 4:30pm (35 hours per week).

**Specialized Knowledge and Skill Requirements**:

- Demonstrated experience working with seniors living with various types of dementia and care partners.
- Demonstrated extensive knowledge and skills in case management techniques including the identification, consultation, and assessment of the high-risk seniors’ population living with dementia and/or care partners.
- Demonstrated crisis intervention, de-escalation, and conflict mediation skills.
- Experience working in an integrated team using a collaborative approach.
- Knowledge of Community Support Services, Primary Health Care and Mental Health systems
- Knowledge of consent and capacity issues and legislation related to seniors living with dementia
- Excellent verbal and written communication skills
- Excellent organizational, time management skills
- Ability to prioritize, problem solve, be flexible, creative, and practical
- Ability to work in a fast pace, multi-disciplinary team
- Ability to work independently in a professional, self-directed manner
- Possesses a valid Ontario Drivers’ license and access to a vehicle
- Clear criminal reference and vulnerable sector check
- Knowledge of PIECES framework and templates is a strong asset
- Proficiency in French language or other language(s) is an asset

**Educational Requirements**:

- Minimum of 2-3 years experience in a Case Management role working with at risk seniors living with dementia and/or care partners.
- University degree in social work (MSW, BSW, SSW), Gerontology or a related field is required.
