**Salary band is $47,759.04 to $58,164.06 annually**

We are currently seeking a** two permanent full-time Peer Support Workers**. Reporting to the Program Manager, Recovery West, the Peer Support Worker will provide holistic assessment and peer support to individuals with mental health concerns. They will carry an individual case load as well as creating, implementing, and facilitating groups and activities within Recovery West.

CMHA Peel Dufferin has adapted to a hybrid working model including a mix of on-site and remote work. Staff are required to have onsite presence 2-3 times, or more if needed, to meet client and/or program needs.

**What you will do**
- Conduct assessment and assist clients to develop goals for recovery using one-to-one and group sessions
- Utilize tools such as the Wellness Recovery Action Plan (WRAP) to assist clients in creating individual care plans
- Support clients in developing empowerment skills and combating stigma through self-advocacy
- Assist to identify and combat negative self-talk and identify and overcome fears by sharing own techniques and strategies
- Manage a caseload including regular re-assessments, updating risk levels and caseload reviews
- Collaborate with clients and the multi-disciplinary team to establish Recovery Plans which guide mutual work towards client recovery
- Facilitate groups as part of CMHA PD’s multi-disciplinary team, including the creation of new groups and act as a back-up facilitator
- Refer and link clients to community resources and groups that support recovery goals
- Participate in regular staff meetings and care planning to provide the peer expertise for client care
- Actively engage clients in the daily activities of the program
- Update all appropriate documentation within 24 hours of client contact, as required by CMHA PD policy and procedure
- Participate in the Peer Community of Practice and other agency and community committees and workgroups as requested by manager

**What you will need to join our team**
- A personal experience with mental health and/ or addiction and established recovery, as well as willingness to share experience
- Post-secondary degree or diploma in relevant discipline (e.g., BSW, MSW, OT, RN)
- Peer certification is an asset
- Being a registered and / or regulated health professional is an asset
- Minimum 2 years’ experience in the mental health and addictions sectors with a focus on group facilitation and recovery oriented practice

- Demonstrate self-motivation to function as part of an interprofessional multidisciplinary team
- Understand the importance of confidentiality and boundaries
- Conduct assessments and coordinate
