**POSITION TITLE : PTH - Mental Health Case Manager**
**REPORTS TO : Pathways to Home (PTH), Program Manager - 1 Year Contract**

Madison Community Services is a non-profit agency, which promotes the independence, health, recovery and community integration of persons with mental health challenges through the provision of mental health case management, supportive housing and a broad range of other programs and services.

Case Manager provides direct service and support to clients in the programs of Madison.
Reporting to PTH, Program Manager, the Mental Health Case Manager is responsible for providing recovery-oriented supports to clients in the following areas: needs identification, individualized assessment and planning, direct service provision, coordination and support, referral, monitoring, evaluation, and advocacy.
**DUTIES, RESPONSIBILITIES, AND RELATED TASKS**
- Assist in the planning, implementation and evaluation of the supports and services.
- Comply with the agency policies, standards, guidelines and procedures.
- Participate effectively and respectfully in staff and other designated internal meetings.
- Work in cooperation with management and other staff members in the provision of efficient and coordinated services.
- Work from a recovery framework in achieving the organization’s mission, which is: _**to promote the health, independence, recovery and community integration of persons with mental health and/or addictions challenges through advocacy, education and the provision of a broad range of community based and housing support services**_.
- Complete assessments that identify clients’ needs, priorities and abilities through information obtained from the client, his/her formal and informal supports and the referral source utilizing agency and funder tools.
- Develop with each client a comprehensive individualized plan, which incorporates his/her goals and values and identifies all skills, resources and service requirements.
- Assessing the needs of clients/tenants and working with them and their supports to develop and implement meaningful housing support plans.
- Provide direct service including: ADL skills coaching, assistance in obtaining and maintaining housing, support, encouragement, counselling and feedback to enable clients to reach their goals, counselling and information to reduce the incidence, duration, intensity and impact of problematic symptoms, family support and education as appropriate to support the interests of the client.
- Maintain confidentiality and comply with agency standards and all applicable legislation.
- Provide information and strategies for health and mental health promotion and crisis prevention.
- Assist clients in identifying and advocating for their civil, legal, housing and other rights.
- Facilitate access to desired and needed services and resources, including community supports, social recreational supports, crisis intervention and treatment, among others.
- Assist clients to identify, build and maintain a social/informal support system, where desired.
- Collaborate with community service providers.
- Participate in and contribute to internal agency committees.
- Perform duties in a safe manner, in line with Madison’s Health and Safety policies and all applicable statutes.
- May assist in the training and supervision of students and/or volunteers.
- Monitor, regularly review and, if appropriate, revise client goals based on client’s individual needs and direction.
- Evaluate achievement of goals and client satisfaction using a variety of methods.
- Maintain client files and documentation as per agency policies, procedures, standards, guidelines and legal framework.
- Collection of data including: client demographics, service statistics, goal achievement and other systems that may be developed.
- Maintain relationships with networks, related professional organizations and other related service providers.
- Represent the mission, values and philosophy of Madison in the community including particular networks or meetings as requested.
- With the support and direction of PTH, Program Manager, develop an annual learning plan to move toward identified performance and professional development goals.
- Participate in regular supervision with PTH, Program Manager
- Attend relevant conferences, workshops and other in-service events as approved and/or directed.
- Become fluent in Madison’s policies, procedures, standards and guidelines of operation.
- Perform other related duties as assigned by Senior Manager Supportive Housing Program or designate in the absence of the Manager.
**KNOWLEDGE AND SKILLS**
- Good understanding of relevant provincial, and local legislation and government policies, procedures and practices pertaining to services provided by the agency.
- Knowledge of the Residential Tenancies Act and the Mental Health Act.
