**The Opportunity**:
Reporting to the Manager with responsibilities for School Health Partnership, the Clinical Practice Leader, SHP works collaboratively with partners within the health and education sector (for example, the DEECD, NSH, CSAP, and the Regional Centers for Education). The Clinical Practice Leader, SHP provides support to the SHP Nurses that work within the School Health Partnership Program. The Clinical Practice Nurse, SHP works to ensure that the standards of practice, code of ethics and relevant health and education policies are incorporated into decision making and processes that impacts professional, interprofessional and intersectoral practice. To foster and advance evidence-based practice, a portion of the Clinical Practice Leader, SHP’s time will provide SHP Nursing services.

The Clinical Practice Leader, SHP provides expert clinical practice support to SHP Nurses, and liaising with system partners to further define and enhance the SHP Program. They plan, develop, and implement educational activities at a team level to foster the development of competence of the SHP Nurse team and partners in education. They participate actively in the development of peers and partners through coaching and mentorship and by sharing knowledge and resources and providing feedback. Provides leadership in the delivery of evidence-based nursing care and facilitates the integration of research into meeting established School Health Partnership standards, guidelines, and protocols. Acts as a clinical resource to the SHP Program related to evidence-based patient care, continuing education, development and/or review of policies, objectives, and guidelines.
- Supports the development and implementation of SHP nurse orientation program, working closely with SHP Managers and with partners in Education (Regional Centres for Education and CSAP)
- Collaborate with others (PPLs, education partners) to identify and resolve practice issues.
- Identify, advocate, and promote educational and/or mentorship opportunities that: develop knowledge, skills, professional competencies and/or critical thinking to enhance professional practice consistent with the priorities of the organization and of the program
- Develop and maintain strong relationships with professional organizations and associations. Builds and maintains a strong working relationship with the IWK and NSH Interprofessional practice program, including the nursing practice offices
- Contributes to/or co-leads quality improvement activities and evaluates outcomes of practice changes
- Collaborates with stakeholders both internal and external to identify and lead process improvement initiatives
- Triages referrals from schools, health providers and services. Referrals are also made between Regional Education Centres
- Confirms the student’s need for school-based health services through a comprehensive assessment including information from parents, school staff and health care providers
- Assesses the level of health and safety risks to student and school staff, in the school-based plan of care
- Develops a needs-based plan of care for students who have been triaged as appropriate for SHP RN care
- Works with school staff and health providers to ensure the student’s plan of care can be safely and consistently provided at school

**Hours of Work**:
Hours of work: Monday - Friday 8:30-4:30, location based out of the HRCE office in Burnside

Travel within HRM will be required

**Your Qualifications**:

- Bachelor or Nursing, Master’s degree preferred
- Current registration with the College of Registered Nurses of Nova Scotia (CRNNS)
