**PSW Program Manager**
**(Assisted Living Services, Home First, Home at Last)**
**(Full-Time)**
At Community Care Durham (CCD), coordinated networks of volunteers and inter-professional staff, working out of integrated community hubs and other locations, strive to enrich the well-being and quality of life for people who choose to live at home. As the largest community support services agency in Durham Region, CCD collaborates with its partners and the broader community to address community issues and support local citizens to achieve their goals to live independently.
Reporting to the Vice President (VP), Quality and Clinical Standards, the Program Manager, Assisted Living Services, Home First, Home at Last manages the provision of services to clients and provides supervision and leadership to the support staff of the programs. Working within government regulations and CCD policies, the Program Manager oversees the effective and efficient operations of the programs to meet reporting and client care requirements.
Responsibilities Include but are not limited to:
- Plans, implements and evaluates the Personal Support Workers (PSW’s) supported services in consultation with the Vice-President to meet client needs
- Follows up with PSW’s, clients and care partners in timely manner to address complaints and concerns for resolution
- Identifies and manages risks associated with vulnerable clients and liaising with Lakeridge Health and community organization in the coordination of services
- Participates in the recruitment, selection, on-boarding and orientation cycle for Personal Support Workers, Homemakers, and supporting staff to meet the organization’s needs
- Provides performance management, supervision, and effective leadership to Personal Support Worker staff, and supportive team
- Identifies gaps in service delivery and care coordination while providing recommendations for efficiencies and quality improvement to Vice-President, Quality, Clinical Standards
- Regularly reviews program needs with Vice-President, implementing feedback, and creating action plans to meet program needs
- Manages fiscal responsibilities through the development of program plans/target adjustments, budgets, administration of financial resources within the approved budgets
- Provides on-going support and training to members of staff as needed
- Liaises with community organizations, local advisory committees, and other organizations
- Actively contributes to organizational policies and strategic planning as it relates to client care/customer service
- Participates in the selection and implementation of systems and procedures to improve efficiency
- Participates in the on-call rotation and is willing to work on weekends and statutory holidays as needed
**Qualifications required**:
- Diploma or Degree in Healthcare, Community Social Services, or relevant educational background
- Nursing or other Healthcare Designation (current registration with applicable Ontario College) is an asset
- Minimum 3 years of experience in management role
- Knowledge of principles and practices of personal support services and scheduling
- Experience in client assessment, team building, and staff relations
- Intermediate level computer skills (MS Office and Intranet)
- Experience working in non-profit, and community care setting is an asset
- Ability to work in a safe manner and promote the health and safety of all staff, volunteers, clients, and the community as outlined within CCD policies
- Satisfactory Police Vulnerable Sector Check
- Sensitivity to adults with needs related to aging, physical and/or mental health

**Compensation**:Starting at $70,000 annually; 3 weeks’ vacation per year of work (pro-rated to hours worked); 9 paid (including stat) holidays; health, insurance and pension benefits when eligible.
