**Job Summary**:
**Duties and Responsibilities**:

- Perform comprehensive assessment of medical and functional documentation in preparation for efficient sick absence claim adjudication by corporate physician and development/review of modified work plans.
- Liaise with medical professionals, employees, insurer and supervisors to discuss, clarify and gather appropriate details to determine issues that may be impacting recovery and return to work. Health coaching is offered to the employee throughout the process.
- Monitor, make recommendations and evaluate employee's progress in the program and make recommendations to support employees' return to full regular duties or permanent alternate placement.
- Coordinate and monitor stay at work/return to modified work plans.
- Assess and coordinate need for additional testing, third party assessment and treatment.
- Perform physical and cognitive demands analysis, industrial and office ergonomic assessments, job matching review for accommodations, and provide ergonomic advice on design of new workspaces and equipment selection.
- Ensure compliance with Human Rights Code, absence policies and practices, LTD contract, Occupational Health and Safety Act, WSIB and various collective agreements. Comply with relevant City policies.
- Support employee wellness initiatives such as mini-webinars, health coaching, employee wellness health fairs and training.
- Provide ad hoc assistance on projects/initiatives within the unit/team.

**Skills and Qualifications**:

- University Degree(s) in a related health professional discipline such as Occupational Therapy, Kinesiology or Nursing and current registration with the appropriate provincial regulated health professional body (e.g. College of Occupational Therapists of Ontario, College of Nurses of Ontario, College of Kinesiologists of Ontario).
- Certificate in or working towards certification as an Ergonomist, Rehabilitation Professional, NIDMAR, Occupational Health Nurse and/or disability management is preferred.
- Minimum 5 years progressive experience in the field of disability management, rehabilitation, case management and/or return to work including coordinating, implementing and monitoring modified work programs, and assisting in permanent placement of workers with a disability, including those with mental illness.
- Knowledge of STD (Short Term Disability) and LTD (Long Term Disability) adjudication processes as they relate to supporting claims benefits decision and facilitating return to work.
- Demonstrated knowledge of disability management principles and practice, both occupational and non occupational. Experience in interpreting medical and functional information, ensuring confidentiality.
- Qualified to perform physical and cognitive demands analyses and ergonomic assessments, including formal job matches. Ability to design and/or evaluate appropriate standardized ergonomic tools for corporate wide use.
- Comprehensive working knowledge of pertinent legislation and standards (e.g. Human Rights Codes, Occupational Health and Safety Act and PHIPPA) as they relate to disability claims management and workplace accommodation.
- Sound professional judgement in accumulating and disseminating confidential information with the ability to effectively communicate.
- Relationship management skills to foster positive relationships e.g. with employees who are recovering from illness or injury, supervisors, HR staff, unions, 3rd party providers.
- Excellent problem solving and critical thinking skills.
- Proficient computer skills e.g. Microsoft Office Outlook, Word, PowerPoint, case management software.
- May require some light lifting of supplies/materials.
- Primarily office environment (hybrid model) with some local travel to client groups' work location for return to work planning, ergonomics assessments or health promotion initiatives.
- Excellent time management skills.

Hourly Rate/Salary: $ 83,485.00 - $ 111,315.00
