- Le CIUSSS de l’Ouest-de-l’Île-de-Montréal est présentement à la recherche d'un(e) technicien/technicienne en travail pour combler un poste permanent à temps complet, à l’Ex-Hôpital général de Lachine et nécessite des déplacements sur le territoire du CIUSSS de l’Ouest-de-l’Île-de-Montréal.
- Au sein de ses CHSLD, le CIUSSS ODIM offre un chez-soi à des personnes du troisième âge (et à d’autres adultes en perte d’autonomie), qui sont admises lorsque leurs proches ne sont plus en mesure de leur garantir un milieu de vie sécuritaire.
- À titre de technicien(ne) en travail social au sein de la Direction du programme de soutien à l’autonomie des personnes âgées, vous serez appelé(e) à intervenir auprès de celles-ci afin de les aider à surmonter des situations difficiles (solitude, décès de proches, handicap, maladie, conflits, etc.) et à vous assurer qu’elles aient accès aux services appropriés.
- Horaire: lundi au vendredi, de 8h à 16h
- Vos fonctions vous appelleront à:
- Évaluer les forces, faiblesses et compétences psychosociales des clients;
- Aider les bénéficiaires à dégager les solutions qui s'offrent à eux et à élaborer des plans d'action, tout en fournissant l’encadrement nécessaire;
- Évaluer l’admissibilité des bénéficiaires à certains programmes sociaux;
- Utiliser différentes méthodes, approches et techniques (entretien individuel, activités sociales, etc.) afin d'intervenir de façon efficace auprès de ces personnes;
- Aider les bénéficiaires à trouver et à utiliser les ressources dans la communauté, y compris l'aide juridique, médicale, financière, au logement, à l'emploi, au transport, etc.;
- Fournir des conseils quant aux activités sociales et physiques dans la perspective de maintenir la personne en situation d’autonomie;
- En collaboration avec un travailleur social et avec l’appui d’une équipe multidisciplinaire, mettre en œuvre un plan d’intervention qui répond aux problématiques rencontrées et aux besoins spécifiques du client en termes de dynamique de vie, gestion du comportement et/ou des émotions, ou autres enjeux;
- Effectuer des rencontres de suivi pour évaluer les progrès et réajuster les interventions;
- Participer à l’évaluation en continu des programmes par une observation attentive du comportement des bénéficiaires et de leurs réactions aux interventions;
- Développer et entretenir des relations avec d’autres organismes intervenant auprès des personnes âgées afin de favoriser une meilleure prise en charge communautaire et assurer une continuité dans les services;
- Participer aux rencontres d’équipe, fournir des rapports d’intervention et colliger certaines statistiques.

- Requirements :- Détient un diplôme d'études collégiales (DEC) en techniques de travail social*;
