Case Manager - Housing First (Full Time, Permanent)

**The Role and Opportunity**:
The Canadian Mental Health Association is looking for a Case Manager for our Housing First (North York) Intensive Case Management program (HFICM). HFICM serves individuals with complex mental health and addiction needs from a recovery
- based philosophy. This team is a multidisciplinary one which will provide individualized supports in the North York community. The aim will be to improve the experience of individuals with complex mental health and addictions issues in accessing housing support, and their utilization of appropriate services across the broader health and social services system. Service is provided in the community.

**Salary**:$54,124 - $62,733**

RESPONSIBILITIES
- Client assessment, supportive counseling, intensive case management, and crisis intervention.
- Assess client’s needs (i.e. housing, daily living skills, social resources, employment).
- Health monitoring, symptom management and liaison with community health services.
- Skills teaching, community integration, and linking with community resources.
- Service care planning: develop, implement and monitor service care plan in collaboration with client input.
- Engagement and advocacy on behalf of marginalized group.
- Carrying a caseload and keeping accurate, up
- to
- date information on electronic database.

SKILLS YOU BRING
- Post-Secondary Degree/Diploma in Social Services or a related field
- Minimum of 3 years full-time experience in direct service delivery to adults with psychiatric disabilities
- Lived experience is an asset
- Knowledge and understanding of the Strengths-Based Model and Trauma Informed Care Principles.
- Experience working with diverse communities
- Familiarity with homeless outreach initiatives and the shelter system
- Ability to work within a multidisciplinary team as well as work independently
- Excellent communication, listening, and interpersonal skills
- Ability to demonstrate a non-homophobic and anti-racism approach
- A valid Ontario Driver’s License, access to a vehicle and insurance coverage.
