Milestone Foster Homes is licensed by the Ministry of Children, Community & Social Services. Our team of professional caregivers & staff provides out-of-home treatment foster care and supported living to individuals from newborn through adulthood. Our 110+ homes are located throughout Ontario.

**Job title: Case Management Clinical Social Worker**

Start date: Immediately

Supervisor: Manager of Clinical Services

Job category: Permanent

Position type: Full-time. First 2 weeks in-office then moves to hybrid with 16 hours in-office each week.

**Responsibilities**:

- Case Management Clinical Support including:

- Review referral documents and incident reports (including Serious Occurrence Reports)
- Safety planning oversite
- Ad-hoc clinical consultations
- Attendance at weekly clinical planning meetings
- Consultation regarding SMART goal development for CYC support requests
- Sourcing resources
- Attendance at school meetings
- Attendance at Plan of Care meetings
- Attend foster homes for observation, consultation and to formulate recommendations, as needed
- Provide trauma-informed foster parent coaching
- Administer the Trauma Symptom Checklist for Children as per TSCC procedures
- Administer the Ages & Stages Questionnaire as per ASQ training
- Support recruiting with understanding any concerns related to onboarding of potential caregivers

**This is a new and evolving position, therefore, the responsibilities listed are likely to evolve.

Qualifications:

- BSW or MSW
- Child welfare experience, ideally foster care
- Mental health experience
- Working knowledge of complex needs (e.g., Trauma, FASD, ASD, ADHD, Depression, Anxiety)
- Ability to work in partnership with caregivers
- Excellent time management skills and ability to multitask and prioritize work
- Attention to detail and problem-solving skills
- Excellent written and verbal communication skills
- Strong organizational and planning skills
- Proficient in MS Office
