**The Opportunity**:
Reporting directly to the Clinical Pharmacy Manager with shared accountability to the Division of Infectious Diseases (ID) and the Antimicrobial Stewardship (AMS) Program and with professional accountability to the Pharmacy Professional Practice Leader, the Clinical Pharmacy Specialist ID/AMS will have direct patient care responsibilities as part of an interdisciplinary team in an inpatient and outpatient clinic based setting and will also provide pharmaceutical care by designing, implementing and monitoring a therapeutic plan based on current evidence-based knowledge to achieve definite outcomes.

The ID/AMS Clinical Pharmacist Specialist will co-lead the IWK Antimicrobial Stewardship Program by employing evidence-based, patient-centered, safe, and effective pharmaceutical care to influence the way antibiotics are prescribed and used in clinical practice. The ID/AMS Clinical Pharmacist Specialist is responsible and accountable for overseeing the development, implementation, coordination and overall effectiveness of AMS standards, best practices, policies and procedures and provides AMS leadership and champions improvement methodologies; coordinates AMS education to prescribers, nurses, pharmacists, students, and other health-care professionals on infectious disease and antimicrobial stewardship related medication management and promotes a culture that perceives AMS as a priority. The ID/AMS Pharmacist maintains reviews of antimicrobial prescribing practices; analyzes clinical processes and develops strategies to maximize AMS and oversees a variety of quality improvement strategies, participates in change management activities to achieve optimal AMS performance and reports related performance indicators to the Antimicrobial Stewardship Program and to the Drugs and Therapeutics Committee. The ID/AMS Clinical Pharmacy Specialist serves as the co-chair of the Antimicrobial Stewardship Committee, subcommittee of the Drugs and Therapeutics Committee, and is a liaison for the Infection Prevention and Control Committee and the microbiology laboratory. Additional responsibilities include, clinical pharmacy resource for the HIV outpatient clinic, optimizing medication therapies, performing the functions of a staff pharmacist as necessary and completing other projects as assigned by the Clinical Pharmacy Manager, or Director of Pharmacy. The ID/AMS Clinical Pharmacy Specialist will also act as a preceptor of pharmacy students and residents and participate in practice-based research activities.

**Responsibilities**:
Responsibilities include, but are not limited to:

- Conducts initial pharmacotherapeutic assessments, develops and executes a comprehensive care plan for management of medication therapy in collaboration with the interdisciplinary team. Provides consultation services to internal and external health care professionals and applies advanced level of theory to assess, plan, implement and evaluate the delivery of pharmaceutical care.
- Provides clinical direction on complex cases, including the use of evidence-based approaches to evaluate drug therapy, attends patient care rounds, reviews medication histories, provides drug information, patient education and discharge consultations and provides recommendations in the planning and treatment assessment.
- Acts as a clinical pharmacy resource as a member of Drugs & Therapeutics Committee, including preparation of materials for formulary decisions and policy-making decisions by evaluating and making written recommendations regarding formulary requests, conducting relevant research, and assessment for presentation at the Drugs & Therapeutics Committee, and providing necessary knowledge translation to the all members of the health care team.
