**Job Overview**:
The Youth Outreach Worker will provide outreach-based services working with youth within their residence and community settings and attend directly and frequently to provide direct services supports. The Youth Outreach Worker will be required to work flexible hours consisting of primarily afternoon, evenings and weekend hours and is expected to connect with youth frequently and as supports are required. The primary roles will be to support the at-risk youth to achieve goals with the plan of care and youth plan to promote safety and wholistic wellness. The Youth Outreach Worker facilitates youth movement through traditional and/or non-traditional supports or emergency services as required to case management, housing, education, mental health and addiction services, skills training, and employment.**Qualifications**
**Education**:
- A Child and Youth Worker Diploma is preferred. A diploma or degree in a related field in combination with related experience may be considered
**Experience**:
- Two (2) years of direct minimum experience with at-risk populations.
- Experience working with youth aged 14-24 including knowledge of harm reduction, safety planning, and crisis intervention.
- Lived experiences as a First Nation community member in the First Nation/Urban community setting.
- Previous work with coordinating and developing services with Indigenous youth is an asset.
**Duties**:
- Guide and support the youth with tasks to achieve the youth’s goals within the plan of care relative to transition skills to young adulthood and youth’s plan through the voluntary agreements with KGCFS.
- Provide service supports through after-hours services at current resource settings and residential youth programs and services.
- Assist with ensuring development of wholistic safety plans and that safety plans developed with the youth are communicated and reviewed on a regular basis and revised accordingly through ongoing review with the youth.
- Guide and support the youth in engagement in services outlined within the youth service plans to achieve wholistic wellness including identifying positive strengths and gifts that the youth carry. A strength-based perspective will be implemented relative to the youth outreach services and ongoing review of the planning with the youth.
- Complete recordings, reports and case management requirements in accordance with all indirect and direct contacts and attend youth plan development with assigned KGCFS service team members and other service and community related meetings.
