Position Summary

**Dietitian, Telemetry**:
Your goal in this position is to provide excellent patient and family driven care, in a supportive and culturally sensitive manner. You will screen and assess patients requiring nutritional care and developing appropriate nutritional plans of care. You will recommend/interpret diet prescription/nutrition support orders by utilizing best practice guidelines. You will chart assessments, plan and progress in the patient’s medical record. You will participate at medical team meetings and as act as liaison and resource person for nursing and medical staff. You will counsel individuals and groups in normal and therapeutic nutrition. You will take on the shared responsibility for ensuring that health and safety policies are followed to provide for a healthy environment for patients, staff, physicians, volunteers and families. You’re a great relationship builder, and foster close contacts with peers across the entire organization. You have great interpersonal skills, and effortlessly connect with physicians, staff, patients, and families. This position supports the Hospital’s academic mandate by advancing care through teaching and learning and/or transforming care through research and innovation. This position contributes to the shared responsibility of ensuring health and safety policies are followed to provide for a healthy environment for staff, physicians, volunteers, learners, patients and families.

On a practical level, you will
- Provide client-centered assessments of emotional, physical, cognitive and social needs and capabilities of patients as well as their families
- Prepare nutrition care plans that meet patient needs in collaboration with the inter-professional teams
- Chart written assessments and progress notes on patient health records; monitor, reassess and evaluate for optimal outcomes, which are communicated to other health care team members, as required
- Recommend and interpret therapeutic diet and nutrition support orders by utilizing best practice guidelines
- Advocate for the patient/family and community to ensure the patients’ rights are recognized and appropriate services are created/provided in collaboration with the inter professional team
- Formulate and facilitate the discharge plan with patient, family, and health care team utilizing appropriate community resources; recognizing current needs, strengths, and limitations of patient and family resources
- Participate in medical team meetings and inter-professional rounds, and acting as a liaison and resource person for all staff.
- Counsel individuals and groups in normal and therapeutic nutrition program
- Support the education program for nutrition students by directing, supervising and evaluating their performance
- Maintain excellent knowledge of and close liaison with hospital & community resources, i.e. Community Care Access Centre, Social Assistance, Ontario Disability program, Drug Programs, etc.
- Promote best practice and evidenced-based care through the implementation of initiatives aligned with the organization’s strategic direction. Support in the development of program/unit goals, objectives, policies, procedures and quality assurance programs
