**Job Overview**:
The Nipissing Wellness Ontario Health Team [NW OHT] was formed in 2019 in response to the Ministry of Health’s restructuring of our provincial healthcare system. To date we are a collaborative group of 30+ member organizations who are working together with patients, families, and caregivers, to enhance healthcare across the Nipissing region.

We are currently seeking a dynamic and experienced Manager of Integrated Home Care to support the design, implementation, and monitoring of integrated home care services within our Ontario Health Team. The Manager will play a critical role in supporting the implementation and coordination of integrated home care initiatives within Nipissing Wellness Ontario Health Team (NWOHT). The position offers an exciting opportunity to contribute to the transformation of healthcare delivery and improve health outcomes for individuals and families across the Nipissing district.

**RESPONSIBILITIES**:

- Work with NWOHT members to support the introduction of integrated home care models within NWOHT
- Stay informed about MOH and OH directives related to home care transition to OHTs and develop and implement strategies that will support this work
- Complete review of existing home care models for indicated population cohorts
- Design, implement and monitor integrated home care models introduced within NWOHT
- Stay informed about best practices, emerging trends, and regulatory requirements in home care and aging-in-place services and implement innovative approaches to improve outcomes and client satisfaction.
- Collaborate with leadership of NWOHT Health Service Providers (HSPs) to support management of home care programs and services across NWOHT
- Support change management efforts through the program implementation period
- Facilitate communication among NWOHT members and other stakeholders
- Maintain awareness of home care regulations and relevant policies
- Participate in provincial discussions related to the transition of home care to NWOHT
- Prepare briefing notes and presentations for NWOHT Collaboration Council members
- Work closely with NWOHT Executive Lead and other staff members to coordinate home care work with other deliverables and projects of the NWOHT
- Embed in all NWOHT work the shared vision, values and guiding principles including principles of equity, diversity, inclusion, and anti-racism.

**QUALIFICATIONS AND EXPERIENCE**:
**EDUCATION**:

- Bachelor’s or Master’s degree healthcare administration, or a related field.
- Minimum of 5 years of experience in home care management, care coordination, or community-based healthcare services, with a focus on integrated care models.
- Minimum of 5 years of project or program management experience within healthcare preferably with community-based projects/programs
- PMP designation
- Evidence of successful implementation of other healthcare related projects
- Bilingualism is an asset
- Excellent leadership, communication, and interpersonal skills, with the ability to effectively engage and motivate teams

**JOB SPECIFIC COMPETENCIES**:

- Ability to successfully manage complex projects involving multiple partners
- Working knowledge of Home Care within Ontario context
- Exceptional interpersonal, written, and verbal communication skills including the ability to demonstrate tact and diplomacy with diverse groups, including patients, front-line healthcare providers, community agencies, and healthcare administrators.
- Ability to write and edit clear, and grammatically correct content
- Ability to multitask and set priorities, managing time effectively
- Ability to manage demands independently in a fast-paced environment.
- Advanced computer skills [Microsoft Office - PowerPoint, Word, Excel, and Adobe products
- Awareness of Ontario Health Teams, and working knowledge of the healthcare system within Ontario, preferably with experience in Northern Ontario
- Awareness of First Nations / Inuit / Metis / Francophone culture and evidence of cultural sensitivity training
- Understanding of, and commitment to, the NW OHT’s vision and values
- Possess a high level of accountability and proven capability to identify opportunities, take initiative, and think through future implications.
- Knowledge of OHT patient partnership strategies
- Awareness of the Provincial Patient Family Caregiver Declaration of Values
- Awareness of the NW OHT Patient Partnership and Engagement Strategy
- Awareness of OHTs and Connecting Care Act [2019]

**PHYSICAL ENVIRONMENT AND WORKING CONDITIONS**:
