**Role Overview**

We require a Supportive Housing Worker “SHW” to join our Collegeview** **team.

In this role the Supportive Housing Worker will assist service users to maintain housing, support skill building, promote community integration and facilitate therapeutic group activities in an effort to build a sense of community. Supportive Housing Workers will provide individual support to tenants through direct engagement, overseeing the program, ensuring the safety, security and wellbeing for all service users.

**About the Team**

Collegeview’s Team is comprised of Supportive Housing Workers, Personal Support Service Worker’s, a Supervisor and a Manager. Collegeview staff team support the program every day**, **24hrs 7 days a week.

Collegeview is a supportive housing program located at the Collegeview Apartment site in the Yonge St./ College St. area of downtown Toronto. This program provides rent-geared-to-income accommodation and supports to adults living with Acquired Brain Injuries requiring various levels of physical, cognitive, and behavioral support. Service Users reside in their own apartments and share a communal kitchen. The goal of the program is to assist tenants with activities of daily living and other individual goals so that they can maintain their housing and lead productive lives within the community.

**Primary Responsibilities**:

- Advocate with and on behalf of clients to improve and or maintain wellbeing.
- Shares responsibility with the Team for implementing appropriate housing supports that assist clients in pursuing/achieving their goals.
- Manage medication and mediate client mental and physical care.
- Support/encourage clients with activities of daily living.
- Support the maintenance of a safe/ clean environment including pest control preparation and support.
- Create, maintain and improve a healthy working relationship with the service user and their circle of care.
- Plan and facilitates group activities/events which foster client engagement/participation, the development of social networks, and skill acquisition.
- Participates in team meetings, provides input and ideas for improving services, efficiencies and ongoing program planning.
- Support the meal program, task include but not limited too: menu planning, meal preparation, grocery shopping
- Work collaboratively with internal and external stakeholders.
- A minimum of a Community College Diploma in a field relevant to the target population. Higher level post-secondary education will be considered an asset
- A minimum of 2 years direct or transferable experience suitable to working effectively with people with ABI in a community-based setting
- Knowledge of community resources relevant to the target population being served
- Ability to design and implement group activities and facilitate group meetings
- Good crisis management and dispute resolution skills
- Good interpersonal and communication skills (verbal and written)
- Well-developed computer skills (Word, E-mail, Internet)
- Proven skills and/or understanding of how to manage challenging behaviors (Current CPI certification is preferable)
- Demonstrated understanding of ABI symptoms and strategies
- Ability to develop crisis safety plans and goal plans with clients
- Ability to provide individual activities of daily living and other instrumental activities of daily living (i.e. cooking, cleaning, laundry).
- Experience working with people with mental health and/or addiction issues is an asset.

**Other Requirements and Working Conditions**:

- Program hours of support 24 hrs daily (3 shifts).
- Accessible via TTC, blue line on holidays and Sundays.
- Parking available.
