**Position Description**:
An exciting temporary opportunity exists for a part-time Care Facilitator for 12 months with the Home Support Services team to coordinate services and support for older adults living in their homes in the City of Waterloo. The program promotes the following programs: transportation, homemaking, home maintenance, congregate meals, and telephone reassurance. This position reports to the Supervisor of Community Support Services.

**Accountabilities**:

- Assess the needs of vulnerable older adults by conducting in home visits as well as on-site assessments
- Coordinate the delivery of home support services for isolated and/or frail older adults
- Conduct intake assessments and implement client care plans
- Respond to client, family and/or care partner inquires about available services, service parameters and concerns
- Liaise with care coordinators from other agencies to ensure optimal service delivery for each client
- Train and oversee a team of volunteers providing services in the community
- Recruit, match and train brokered workers providing Home Support services
- Assist in the development of new service initiatives for Home Support Services
- Maintain files and statistical information related to Home Support Services
- Enter data for client services in AlayaCare software system
- Provide support to team members and assist with workflow and coverage
- Encourage and validate client strengths’ and abilities
- Uphold the dignity of all individuals while supporting their unique needs
- Maintain confidentiality of client files and records
- Assist with special events related to Home Support Services
- Other duties as assigned

**Minimum Qualifications**:

- Diploma in Social Services, Gerontology or Adult Development, with course completion in the areas of community services and/or healthy aging
- Minimum of one (1) year experience working in a similar position, completing comprehensive intake assessments, service matching, resource navigation and advocacy for older adults residing in the community
- Strong knowledge Home and Community Care and other Community Support Services in the Waterloo Region
- Knowledge of the unique needs of older adults with mental health concerns, cognitive impairments and/or physical disabilities
- Completion of or pursuing completion of a Geriatric Certificate Program would be considered an asset
- Demonstrated excellence in communication, assessment, documentation and organizational skills
- Experience training and supervising volunteers in a community setting
- Experience with Alayacare and/or Caredove would be an asset
- Knowledge of the needs of vulnerable populations
- Knowledge of the community health care system in Waterloo Region
- Ability to work on a cross-functional team
- Proficient in Microsoft Office
- Current Standard First Aid and CPR with AED certification or willingness to obtain
- Valid MTO ‘G’ class driver’s licence in good standing with access to reliable transportation for business use purposes is required to perform the job
- A satisfactory police Vulnerable Sector check is required as a condition of hire
