Community Health Outreach Worker

**The Position**:
Reporting to the Community Health Programs Manager, the Community Health Outreach Worker’s primary responsibility will be to work with an innovative and challenging project regarding HIV, HCV, STBBI’s and harm reduction programs and services. The role will achieve the objectives of the project through community readiness, community development, and capacity building activities. The Community Health Outreach Worker will support rural and First Nations communities through the process of preparing, planning, and implementing HIV, HCV, and STBBI’s and harm reduction education, programs and services, in addition to providing occasional support to the broader STC Community Health team.
**Responsibilities**:
- Demonstrate behaviors and attitudes that represent the Code of Ethics and overall spirit of STC Health and Family Services Inc. as a place that is judgement free, accepting, compassionate and empathetic regardless of race, nationality or economic standing.
- Work with the rural and First Nations communities to carry out education and awareness activities of the HIV, HCV, STBBIs and harm reduction project.
- Create and develop HIV, HCV, STBBIs and harm reduction resources for rural and First Nations communities, organizations and professionals to implement as services and education.
- Provide rural and First Nations communities, organizations and individuals with information sessions, presentations, workshops and learning opportunities relating to HIV, HCV, STBBIs and harm reduction programs and services.
- Develop relationships with rural and First Nations communities, organizations and professionals who would like to learn or implement HIV, HCV, STBBIs and harm reduction programs and services.
- Collaborate and network with rural and First Nations communities, organizations and individuals across Saskatchewan on HIV, HCV, STBBIs and harm reduction.
- Provide guidance and support to rural and First Nations communities, organizations and individuals interested in the process of implementing or improving HIV, HCV, STBBIs and harm reduction programs and services.
- Support the coordinator and manager in planning and coordinating education and awareness activities for the rural and First Nations communities including document preparation, communication activities and coordinating with staff, community members and personnel on site.
- Work with elders, youth and peers to engage groups in the project.
- Manage multiple and diverse tasks.
- Participate in program evaluation by tracking and documenting program activities for reporting and evaluative purposes.
- Participate in accreditation program, maintaining and achieving best practices and quality improvement.
- Respond to community to provide emergency support as a member of the Critical Incident Stress Management (CISM) team, if qualified/needed.
- Adhere to STC Health & Family Services Inc. policies and procedures.
**Education and Experience**
- Post-secondary education in Social Sciences, Education or equivalent is a requirement;
- Must have a minimum of 2 years’ experience in a related position.
**Knowledge, Skills, and/or Abilities**:
- Knowledge of HIV, HCV, STBBIs, and harm reduction;
- A creative problem solver, resourceful and comfortable working in a flexible and challenging work environment.
- Knowledge of the social determinants of health, especially as it relates to First Nations people;
- Experience networking, creating and maintaining relationships with potential partners, community members and professionals;
- Experience facilitating group planning meetings/sessions to meet targeted goals required;
- Experience developing presentations and materials for learning sessions, workshops and conferences;
- Must be a strong communicator with excellent presentation and written communication skills;
- Knowledge of Community Readiness Model is an asset;
- Knowledge of the political, social and economic objectives of the Saskatoon Tribal Council and of the Dakota, Cree, and Saulteaux cultures will be considered an asset.
**Other Conditions**:
- Some services provided in community setting which present many unknown variables;
- Ideally will travel out to communities multiple times per week (COVID19 has altered this);
- Exposure to clients who may have impaired judgment due to emotional distress, communicable diseases, mental illness or intoxication;
- Some heavy lifting may be required on day-to-day basis.
- Hours of work may vary, must be willing to travel and work flexible hours, evenings and weekends, and to perform other related duties as required or assigned;
- Must possess a valid Saskatchewan driver’s license, a reliable vehicle and meet STC’s insurance requirements;
- Must provide a current, original, Canadian Criminal Record Check (CPIC) and vulnerable sector check as a condition of employment;
