**Role Description**

**Position Summary**:Behavioural Supports Ontario (BSO) provides enhanced, integrated, and cross-sector services that meet the needs of older adults with responsive behaviours associated with dementia, mental health, addictions, and other neurological conditions, in the community, long-term care (LTC) or hospitals and
Community. The Erie St Clair BSO framework for the LTC homes asks that there be a collaborative approach to supporting responsive behaviours, thereby increasing their quality of life. Integrated services include System
Navigators, LTC Internal Champions, LTC Lead Teams, Geriatric Mental Health Outreach Teams (GMHOT),
Hospital Navigators, Clinical Care Coordinators RN’s, Primary Care Providers, and Psychiatrists. The focus for this role is on Long Term Care, Retirement Homes and Community.

**The Psychogeriatric Resource Consultant (PRC)** has a strong knowledge base of behavioural support and the management of complex mental health needs, dementias, and related responsive behaviours. The PRC is responsible for developing and maintaining partnerships with care providers within long term care, community, and hospitals. The PRC collaborates with care providers, families, other specialists, consultants, and community teams to improve the quality of life of clients and their families. The primary role will support in Chatham-Kent involves the capacity building of health service providers, through education, training, and clinical consultation; and providing a leadership role in the development and evaluation of treatment plans.

- Proficient computer skills. (Microsoft products, Zoom, etc.)
- Comfortable in a fast-paced environment.
- Fulsome understanding of health equity principals, inclusive of cultural humility and sensitivity

**Qualifications**:

- Completion of a degree in Nursing
- Registered or eligible for registration with the appropriate professional college
- Minimum two years' experience in psychogeriatrics preferred, otherwise significant experience in geriatrics, long-term care or community-based seniors care is essential.
- Experience working directly with seniors' mental health issues, including Alzheimer disease and related dementias, in a variety of sectors.
- Experience in program development, research and evaluation, community development, and education
- Experience with psychogeriatric screening, assessment and development of intervention strategies
- Competence in the use of standard assessment tools
- GPA and/or P.I.E.C.E.S. training preferred.
- Minimum 3 years’ experience working in long term care setting, or with a geriatric population including those with dementia.
- Knowledge and ongoing practice of person-centered care.
- Valid driver’s license.

**Hours of Work**:

- 21 Hours a week between 8:30 and 4:30 pm
- Flexibility in work hours required, some evening and weekends.

**Work Conditions**:

- Physical ability to fully participate in meetings / case conferences in multiple health sectors (i.e. Hospital,

LTC, Retirement Home)
- Extensive travel throughout CK area.
