Founded in 1960, the Youth Services Bureau of Ottawa (YSB) is one of the largest and most comprehensive non-profit agencies in Ottawa and a registered charity. It provides mental health services, employment programs, emergency shelter, community, health and housing services, and justice programs to youth in the Ottawa area, serving between 2,500 and 3,000 at‑risk individuals aged 12+ and their families every month. YSB employs 350 caring professionals in a unionized environment in over 26 locations across Ottawa.

The Wraparound Facilitator, under the direction of the Coordinator of YFCS, provides case management and assessment services, through the Wraparound process, to high need families referred through the Coordinated Access and Referral Committee. All work is done in accordance with policies, procedures and directives established by Bureau.
- Provide case management within the established framework of Wraparound Principles
- Assess client need and recommend plan of service
- Assess families/clients focusing on assisting clients in identifying strengths
- Support clients to identify short and long term goals & strategies for goal achievement.
- May be asked to present family treatment plan to Coordinated Access and Referral Committee
- Prepare for and facilitate meetings with stakeholders in the wraparound process
- Provide follow up support to parents as required
- Maintain current knowledge of community resources
- Advocate for appropriate services on behalf of client
- Ensure timely completion of required documentation
- Attend and participate in supervision & clinical group supervision as required
- Utilize recognized interventions to support client in the achievement of established goals
- Follows safe work practices as established by the Bureau

**Job Specifications/Qualifications**:_(The qualifications are appended to the job description and are not intended to be part of the job description)_
- Degree or diploma in Social Work or relevant field
- Minimum two years’ experience in group work, case management, coordination of services and partnerships with community resources
- Trained in and experienced with Wraparound facilitation process
- Experience with adults and children and/or Youth, with some experience with children under age 12
- Excellent presentation and facilitation skills
- Demonstrated experience with case management
- Demonstrated assessment skills
- Demonstrated decision making and problem solving skills.
- Familiarity with community services and agency protocols for referral
- Oral and written fluency in English is essential
- Oral and written fluency in French is essential
- Availability and ability to work evenings and weekends, flexible hours.
- Demonstrated computer skills
- Valid driver’s license
