The Wetaskiwin Primary Care Network (WPCN) seeks to fill a 0.4 - 1.0 FTE Health Navigator position. This position has the possibility of being a 1.0 FTE, if combined with the 0.6 FTE LPN position. (See LPN posting on our website). The Health Navigator provides patient intake, assessment, planning and navigation for those referred to the PCN for health services. An integral part of the continuity of care continuum, the Health Navigator uses their exemplary knowledge and clinical skills to ensure patients referred for mental health intervention or chronic disease support, are connected with the appropriate member of the interdisciplinary team and/or the correct community resource.
- Is a Licensed Practical Nurse or any other registered health professional, with a minimum of 2 years mental health experience in a clinical environment.
- Has an equivalent combination of education and experience, that may be considered.
- Has training and certification in health behaviour change methodologies, mental health first aid and other relevant professional development.
- Has recent experience in family intervention, social work, or a related human service field.
- Has an active Registration as a member in good standing with relevant licensing body.
- Experience in using electronic medical records is an asset.
- Possesses excellent relationship building, communication and interpersonal skills.

This position reports to the Clinical Manager of the Wetaskiwin Primary Care Network.

**Job Types**: Full-time, Part-time, Permanent
Part-time hours: 25-40 per week

**Salary**: $26.45-$34.63 per hour
