**Summary**: In accordance with the Mission, Vision and Values, and strategic directions of Providence Health Care, the person promotes a safe, respectful, and civil working environment for patients, residents, families, visitors and staff.

Reporting to the Professional Practice Leader (PPL) Social Work, and working collaboratively with the Research, Education and Practice Coordinator, the position supports the interdisciplinary program/team in achieving program, patient and resident goals. The Social Work Site Leader is responsible for the coordination of day-to-day operations within Social Work in designated program areas. Coordinates work assignments, monitors patient flow and acuity and makes adjustments as needed ensuring staffing levels are appropriate. Acts as a clinical resource to staff, students and the interdisciplinary team. Identifies learning needs and interests, participates in development of educational and orientation information and presents to staff as required. Coaches, mentors and supports staff and facilitates effective problem solving as needed. Performs direct patient/resident assessment and treatment as needed. Ensures PPL and Research, Education and Practice Coordinator are made aware of issues or concerns at the site(s). Promotes research and quality improvement initiatives at the designated site(s).

**Qualifications / Skills and Education**: Education
Masters in Social Work plus four (4) years broad experience in a health care setting, including previous supervisory and administrative experience, or an equivalent combination of education, training and experience. Current full registration with the British Columbia College of Social Workers.

**Duties and Responsibilities**: Oversees the day-to-day activities of social work staff at the designated programs by performing duties such as coordinating work assignments, establishing priorities.
Identifies training and education needs of site staff and provides orientation to new employees.
Monitors patient/resident acuity and flow on the designated site(s), and ensures staffing levels meet the unit needs. Assists staff in prioritizing workload. Adjusts assignments as changes in acuity and staffing require and provides clinical support to staff as needed.
Acts as a clinical resource to staff, students, and other members of the interdisciplinary team, by demonstrating procedures, answering questions related to clinical practice issues, and troubleshooting concerns. Advises and works with PPL, Research, Education and Practice Coordinator, and/or Program staff to resolve issues/concerns.
Oversees the day-to-day supervision of designated staff by providing advice/guidance, facilitating resolution to workplace issues and participating in performance evaluations. Acts as a positive role model for staff and refers issues to PPL as required.
Gathers information on staffing changes or requests for coverage, and in conjunction with PPL or designate, allocates staff based upon skill set and unit or site needs, in accordance with applicable collective agreements.
Provides advanced therapeutic clinical counseling services to patients/residents and families in accordance with professional standards and clinical policies and procedures.
Maintains patient/resident records by methods such as documenting patient/resident assessments, charting patient/resident information, preparing progress notes and reports in accordance with established standards, and policies and procedures to meet regulatory requirements and document for future reference.
Collaborates with interdisciplinary care team members regarding Quality Improvement (QI) initiatives, by developing QI projects, implementing processes and procedures upon authorization, and evaluating results. Ensures staff are aware of changes and updates once established.
Provides input into the development, evaluation, and delivery of orientation, in conjunction with the Research, Education and Practice Coordinator and other members of the health care team. Makes recommendations regarding changes or new initiatives and provides site orientation to employees as required.
Identifies learning needs of staff and communicates to PPL and Research, Education and Practice Coordinator. Coordinates and develops education strategies and programs for the site(s). Promotes educational opportunities to site staff and encourages continuing education. Presents in-service educational sessions for site staff as required.
Develops and reviews policies, procedures, practice standards, and reference materials, in collaboration with PPL and Research, Education and Practice Coordinator.
Maintains a variety of records such as workload statistics by methods such as recording information into approved formats, collating data, analyzing trends, accounting for variances, and recommending an action plan to the Professional Practice Leader, Social Work.
