As a Social Worker in the Emergency Department and In-Patient, you’ll engage with a diverse range of patients, many of whom are experiencing acute crises. Your expertise in rapid psychosocial assessment and emergency intervention will be essential in helping patients and families navigate the immediate aftermath of traumatic events, offering them the support and guidance they need in high-pressure situations.

For example, you might guide a family through the stress of a difficult diagnosis, offering resources and emotional support that helps them cope and adapt with clarity and understanding. Your expertise in crisis intervention and brief therapeutic interventions allows you to provide immediate, effective support while respecting professional boundaries and the limits of your role
You are also a creative problem-solver, adept at multi-tasking and quickly assessing situations to implement appropriate interventions. This might involve addressing a patient's immediate needs during a crisis, such as helping a family find temporary support following an unexpected medical event. Your ability to think creatively ensures that patients receive the support they need in critical moments.
**About the Department of Social Work**
Our team provides range of social work services including assessment, crisis intervention, counselling; bereavement counselling and comprehensive discharge planning with individuals and families. Our team is integral to Mount Sinai Hospital’s multidisciplinary approach, ensuring that each patient’s psychosocial needs are addressed alongside their medical care. We are skilled in navigating complex cases that often involve coordinating with multiple departments, and we take pride in developing tailored care plans that support patients through their entire hospital experience and transition back to the community.
**About the Emergency Department**
Mount Sinai Hospital's Emergency Department is dedicated to delivering family-centered emergency care to the community, with approximately 61,000 annual visits.
**In this role you will**:
- Liaise with the Local Health Integration Network (LHIN) regarding service provision, program initiatives and health care concerns
- Work collaboratively and have the ability to connect with multiple teams and team members in this busy teaching hospital
- Provide a range of social work services including psychosocial assessments, crisis interventions, care planning, comprehensive discharge planning and counselling with individuals and families
- Liaise with community agencies to ensure coordination of appropriate services for patients
- Advocate on behalf of patients and families
- Participate in professional development, teaching and research
- Job Requirements**Job Requirements**:
Mandatory
- Master's Degree in Social Work
- Current registration with the Ontario College of Social Workers and Social Service Workers
Preferred
- Previous Social Work experience in a hospital setting
- Experience with crisis counselling, brief counselling models and discharge planning
- Experience in an emergency department
- Experience with issues, addictions, domestic violence, sexual assault, child protection, in addition to experience working with older adults
Skills and Knowledge
- Demonstrated ability in individual and family counselling
- Knowledge of community supports and resources
- Demonstrated ability to work independently and in a team environment
- Excellent interpersonal, verbal and written communication skills
- Demonstrated interest in education and research
- Demonstrated satisfactory work performance and attendance record
