Activities Co-ordinator

Full Time hours are available and require additional program facilitation (Act Aide), including some evening hours worked)
**Requirements**:
- Post-secondary education in Recreation Therapy or Therapeutic Recreation. Social work or Social Service work.
- Behavioral Supports Ontario implementation skills/experience is an asset
- Minimum one (1) year experience in health care setting.
- Minimum two (2) years’ experience in program planning, implementation, documentation and evaluation in a senior’s care setting.
- Excellent communication and leadership skills.
- Proficiency in Point Click Care, RAI-MDS and care planning.
- Passion and focus in respect to the resident centered approach and working with seniors.
- Demonstrated leadership, organizational skills, creativity and high level of motivation are required.
- Valid Police Records Check (within 6 months)
**Responsibilities**:
- Plans, organizes, implement and directly provide appropriate resident centered activities/special events and restorative care programing.
- Is knowledgeable of the Resident Bill of Rights/Governing Principles and skills to oversee it's promotion and implementation.
- Ensures resident satisfaction and continuous quality improvement initiatives are promoted.
- Promotes a positive team spirit within the department and the facility with residents and co-workers.
- Supports MOHLTC Standards and Morriston Park Nursing Home Policies.
- Provides all new residents with a thorough orientation to the Activation Department, including admission required documentation.
- Participates in team conferences to plan, develop and assess individual resident choices and care plans.
- Identifies and encourages opportunities for residents to share special skills or knowledge with others, both inside and outside the facility.
- Maintains the department annual operating budget.
- Insures the operation of the department within approved budgetary parameters.
- Finds financial and physical resources to support departmental services. (Donations for Raffles etc.)
- Maintains appropriate department records including assessments, resident activity response/feedback detailing individual participation. Maintains awareness of Residents conditions and identifies any changes to other appropriate team members.
- Acts as a liaison between facility and residents at Residents' Council & Family Council meetings.
- Record and report to Administrator resident council agenda and outcomes via council meeting minutes.
- Volunteer co-ordination
- Hands on support with residents care needs (portering/meal service)
- Other duties as required.
**Job Types**: Full-time, Part-time, Permanent
**Salary**: $22.88-$26.50 per hour
Schedule:
- 8 hour shift
- Monday to Friday
- Weekends as needed
