Description:
- Reporting to the Nursing Care and Service Manager, the Clinical Office Supervisor will be based in the operational center of the district and will use clinical knowledge and judgement to problem solve and support clinical care and service delivery in collaboration with Clinical Community Supervisors. The Clinical Office Supervisor will have the knowledge to assess, advise on, and evaluate a wide range of care and service delivery situations identified by field staff, clients/families and internal office staff ensuring safe, quality, ethical care provision, and is expected to practice in accordance with regulatory legislation and standards as determined by their regulatory body and ParaMed.- Qualifications:
- Preferably has a minimum of 2 to 3 years’ experience as a practicing Registered Nurse/Registered Practical Nurse
- Registration/licensing with provincial regulatory college in good standing
- C.N.A Certification as a Community Health Nurse is an asset
- Knowledge of clinical documentation standards and practice guidelines is required
- Knowledge of APR clinical documentation and reporting process an asset
- Comprehensive patient health assessment skills are required
- Working knowledge of applicable legislation, including but not limited to, Mental Health Act, Substitute Decisions Act, Healthcare Consent Act, Privacy / Personal Health Information Act
- Superior oral and written communication skills to effectively interact with staff, clients/families, and external stakeholders
- Effective critical thinking and problem-solving skills
- Able to provide a current, clear Vulnerable Position Screening (VPS)
- Demonstrates commitment to professional development in clinical practice that is relevant to home health care
- Responsibilities:
- Responds to and advises/resolves concerns from field staff, clients/families, coordination, public, and funding staff
- Reviews nursing referrals and/or medical orders prior to assignment
- Transcribes telephone clinical orders and distributes to clinicians in the field as necessary
- Initiates client safety incident investigations and documents to ensure timelines are met
- Works collaboratively with nursing team members to support clinical APR documentation and submission requirements according to WWLHIN APR reporting guidelines
- Manages the daily APR reporting and tracking process
- Engages client in problem solving using a collaborative approach, offering health teaching, building rapport and trust, achieving a mutual understanding and agreement to identify actual and potential client health care needs, strengths, capacities and goals
- Engages staff in problem solving offering education on practice questions, coaching and mentoring in resolution of problems
- Maintains the knowledge, skills and attitudes to provide safe, efficient, effective service in all areas of practice by keeping abreast of changes in practice setting that affect scope of practice
- Documents all client care and staff problem solving as per ParaMed policies
- Other duties as assigned
- Competencies:
- Incorporates federal and provincial laws and regulations, national and provincial standards to provide high quality, safe and ethical care to clients/patients i.e. Regulated Health Professionals Act. Assumes professional responsibility, thinks critically, demonstrates practical knowledge, engages in professional development, manages own practice and advocates within system on clients/patients’ behalf.Time Type:
