**Position Summary**

The Community Health Navigator (CHN) works within the Mosaic PCN, targeting patients with ineffectively controlled diseases to improve access to health care and other community resources. This role is the liaison between patients, communities, physicians, and the multidisciplinary team. These patients have multiple challenges and barriers to chronic disease care and the CHN connects these patients with the broader health care system in Calgary.

**Key Responsibilities**

System Navigation/Social Support
- Support patient attachment through regular, patient-centered follow-up, including home visits to engage and empower patients
- Identify appropriate and credible resources responsive to patient needs taking into consideration culture, language, reading level and health literacy
- Coordinate patient care between multiple health and social providers within the MPCN catchment area and leverage MPCN programs and services
- Support attendance, and facilitate access to transportation services to and from scheduled appointments, as necessary
- Provide and/or coordinate interpretation and translation services with patients, as necessary

Patient Advocacy
- Support the development of patient-centered goals, taking into account each patient’s values, preference and circumstance
- Demonstrate responsiveness to patient needs within scope of practice and professional boundaries
- Encourage active communication between patients/families and health care providers to optimize patient outcomes
- Use goal setting and motivational interviewing techniques to encourage positive changes for patients
- Demonstrate empathy, integrity, honesty and compassion in difficult conversations
- Support patient self-management techniques

Inter-professional Collaboration
- Integrate with the clinic team and serve as liaison between clinics and patients
- Promote services within clinics by regularly connecting with physicians and CDM team members to communicate program and patient updates
- Understand the goals related to MPCN health outcomes and support the implementation of plans
- Provide timely follow-up for physicians and CDM team on patients’ status
- Collaborate with University of Calgary ENCOMPASS research team to identify key successes and challenges and support the development of innovative approaches to patient care

Administration
- Maintain timely and accurate records capturing patient interactions, barriers and other pertinent information
- Organize time and priorities effectively as work is remote, in the community, or in patient homes, and may require travel to multiple sites in a day

Continuous Professional Development
- Participate in Community Health Navigator core training
- Identify and work to achieve own professional development needs which supports goals set in performance reviews

**Qualifications & Requirements**
- High School Diploma or GED
- At least 3 years’ community involvement (formal or informal) that reflects strong ties to a local community
- Experience working with different socio-economic groups and backgrounds
- Strong understanding of the health care and social resources available in Calgary
- Desire to help and connect vulnerable patients to resources and supports
- Cultural safety
- Organization and prioritization skills
