**Volunteer Program Facilitator ~ 4 hours per week with possibility of being increased**

The CCHC inter-professional team provides primary health care, health promotion and prevention programs and services to residents of Central Elgin, St. Thomas and Southwold
Township.

_**this is a volunteer position**_
- CCHC is looking for an individual with experience and expertise in recreation for seniors and a passion and strong desire to make a change in the lives of seniors. Reporting to the Program

Mental Well-Being programs which include facilitating social, recreational, and health promotion activities and special events for seniors.

**Primary Responsibilities**
- Develops direction and materials for the _Seniors Mental Well - Being_ program
- Prepares and plans activities and events
- Creatively contribute ideas for new projects, activities, and programs
- Set up and takedown of program space as required
- Documents qualitative, and potentially quantitative feedback from participants
- Provides support to participants as needed
- Assists in the promotion of the program
- Ensures all duties are performed in a safe manner; reporting any items and/or issues that need attention to supervisor and/or Joint Health & Safety Committee
- Participates on committee work as assigned by the Program Coordinator
- Performs other duties as may be assigned by the Program Coordinator or designate

**Qualifications**:

- 2 to 3 years program facilitation experience with seniors
- Experience in gerontology, recreation and leisure, social services, community services, or similar field of study.
- Proficiency in the use of computers, various software including Microsoft Office Suite, and office equipment
- Ability to work in a fast paced environment; maintain a professional attitude and project a welcoming attitude.
- Experience with reporting software required by the LHIN and MOHLTC is and asset.
- A second language would be an asset.
- Experience working with volunteers, seniors, and adults with disabilities, and with community social and health services.
