Health Sciences North (HSN) is seeking an innovative leader for the position of Coordinator to lead the implementation of Bundled Care and Surgical Virtual Care pathways. The Coordinator will engage stakeholders in the development of partnership agreements, influence innovation, and support implementation of new care delivery models. In addition, the coordinator will lead the development of identified improvement opportunities.

**KEY FUNCTION**:
Lead the implementation of Bundled Care and Surgical Virtual Care pathways. Engage stakeholders in the development of partnership agreements, influence innovation, and support implementation of new care delivery models. Ensure robust data gathering and reporting mechanisms and lead the development of identified improvement opportunities.

**REPORTING**:
Under the general direction of the Clinical Manager, Outpatient Rehabilitation Programs.

**DUTIES**:

- Support the implementation of new bundled care models (e.g. Total Joint Replacement, Shoulder Arthroplasty, Coronary Artery Bypass Grafting (CABG) and Stroke).
- Support the implementation of surgical pathways monitored though the Surgical Virtual Care model.
- Partner with community organizations to ensure patients have access to the full scope of bundled care service; ensure contracted out services are being delivered.
- Develop, monitor and adjust agreements with partner providers.
- Provide ongoing communication and education to partners to ensure alignment with best practice guidelines.
- Ensure required clinical and financial data is collected and reported to support outcome reporting, evaluation and protection of financial stability.
- Support participation in the provincial Patient Reported Outcome Measures (PROMS) initiative and the central evaluation of bundled hip and knee Quality-Based Procedures (QBP).
- Responsible for appropriate accounting processes such as service contracts, billing and data report requirements.
- Support successful implementation of anterior hip and same day knee patient interventions.
- Initiate construction of a robust strategic surgical database and optimization of surgical wait time data.
- Evaluate patient care and satisfaction on an ongoing basis and formulate solutions for improvement.
- Guide, motivate, monitor and supervise staff, develop standards of performance, evaluate performance and make recommendations on disciplinary action as required.
- Oversee the work of others in compliance with the Occupational Health and Safety Act (OHSA), its regulations, and HSN policies.
- Determine and align improvement projects with HSN’s Strategic Plan; monitor and adjust to achieve goal outcomes.
- Participate in the accreditation process and work to ensure that the program and HSN achieve, maintain and continually improve upon their accredited status.
