The role of the Community Outreach Support Worker is to assist each participant in our outreach programs by creating a safe environment where the participants are supported to increase their awareness of healthy decision making, utilize life skills and implement coping strategies learned through their therapy and programming. The Community Outreach Support Worker will maintain all outreach records and do monthly case plans and goal setting with outreach participants as well as assist them in tasks within the community.

**General Duties include**:

- Consider and adhere to risk management and participant protection policies and procedures
- Maintain proper procedures and paperwork; ensuring ethical guidelines
- Adhere to privacy and confidentiality agreements
- Follow the mandate of the organization
- Participate in team approach to problem solving and goal setting

**Primary Duties include**:

- Conduct monthly 1:1 case management meetings with all assigned clients
- Help develop curriculum and programs for outreach programs
- Liaise with therapists, doctors and addiction counsellors to ensure continuity of care
- Support Outreach program development, policies and procedures
- Facilitate monthly Outreach meetings, workshops and programs
- Develop and maintain a follow care client handbook
- Provide safety planning and lifestyle awareness information to clients
- Conduct wellness checks on outreach/follow care clients weekly and provide support and referrals as needed
- Participate in outreach activities and delivery of practical needs
- Facilitate referrals to healthcare providers, educational services and other community resources
- Collect data to help evaluate the success of programs and identify needs for new programs
- Input client notes within 24 hours of shift
- Work in collaboration with community partners and maintain positive working relationship with partners and agencies
- Connect in person with care clients as needed ensuring that their transition to independence is supported

**Requirements**:

- College diploma in social work or nearing completion
- At least 2 years of experience working in a social work environment
- Valid G licence and access to reliable vehicle
- Ability to obtain a Vulnerable Sectors Police Check
- Strong communication skills - both verbal and written
- Ability to listen and provide guidance as needed

**Job Type**: Part-time

**Salary**: $21.23-$23.20 per hour

Expected hours: 15 per week
