**Summary**:

- Providing Social Work intervention in a care partner education pilot project, this role works within a team based model to qualifying McCormick Dementia Services clients and caregivers in the form of assessment, case management, education, and making appropriate referrals to other health care providers and social service agencies at the end of the intervention.
- Dementia care partner training occurring in the individual’s home with the person they care for.
- Temporary 12 to16 weeks, part-time (approximately 2 days/week).

**Job Requirements**:

- Bachelor or Master’s Degree in Social Work required
- Successful registration with the Ontario College of Certified Social Workers and Social Service Workers required
- Minimum of 3 years’ experience in the long term care or community support service sector
- Experience in counselling /supporting clients with dementia and their families/caregivers
- Experience in dealing with issues around client transitions and adjustment (eg. admission, discharge, transfer, change in health status and function, decline in independence, etc.).
- Experience in facilitating and leading support groups
- Knowledge of relevant community support services
- General knowledge of the Public Guardian and Trustee Act, Powers of Attorney and Advanced Care Directives
- Ability and knowledge to work within a computerized environment
- Excellent organizational, interpersonal and communication skills
- Ability to work with all levels of the organization
- Demonstrated commitment to relationship-centred care
