About Us
At AgeCare, our mission is to empower residents to live their lives to the fullest by prioritizing their health and well-being. Working in long-term care is more than just a job; it is a calling for many of us.

The Social Worker provides services which include a wide range of planned interventions, financial/legal assistance, adjustment support, consultation, counseling, education, and referral following a comprehensive psychosocial assessment.
What Is In It For You:

- Contacts resident and family following admission to set up an initial interview session.
- Engagement - establishes a rapport with the resident and/or family to enter a helping relationship.
- Conducts interviews, establishing its purpose and communicating empathy, support, understanding, genuineness, trustworthiness, and respect to develop a supportive and therapeutic relationship.
- Utilizes various techniques including reflecting, paraphrasing, clarifying, informing, summarizing, assuring, and interpreting to gather information.
- Outlines the boundaries of the therapeutic relationship with discussion of Social Worker availability, confidentiality and sharing of personal information.
- Attempts to establish a therapeutic relationship of trust where the resident and/or family feel comfortable in sharing personal information related to the resident’s history, which will form the foundation of the psychosocial assessment.
- Life Review including early life history, education, family of origin, work, relationships, and interests.
- Identification of major life events: achievements, losses, moves, illness, deaths, and the coping style utilized by the identified resident and key family members.
