**Position Summary**

The Social Worker practices in a multidisciplinary team to support patients within Mosaic Primary Care Network to improve social functioning and address barriers to wellbeing. They are responsible for delivering a program of assessment, intervention, and consultation in accordance with evidence-based and professional standards of practice. This position involves the provision of services and educational groups to patients across the lifespan, attending to a broad continuum of needs and concerns, and liaison with community and health services to support coordinated referrals and provide advocacy for individuals experiencing barriers to access. The Social Worker supports collaborative working relationships with family physicians and the multidisciplinary team, including regular case consultation. The Social Worker also participates in ongoing quality improvement activities, program development, and evaluations projects to support the continued refinement and delivery of high-quality clinical care.

**Key Responsibilities**
- Performs general psychosocial and strengths-based assessments including: Relevant history, social and economic context, strengths, barriers, and goals.
- Provides comprehensive information on the range of resources and services available to meet patient-identified needs (ex. Social benefits programs, basic needs resources).
- Facilitates routine screening, risk assessment, and intervention for risks including suicide, abuse, and child welfare concerns.
- Provides supportive counselling to attend to emotional needs arising from difficult life transitions, chronic illness, and interpersonal stressors.
- Provides ongoing monitoring and follow-up to ensure patient needs are met by recommended interventions and resources
- Provides advocacy to support equitable access to available resources and assist vulnerable patients with barriers to self-advocacy in areas of social benefits, immigration, and housing.
- Supports positive working relationships and partnerships with community-based non-profits to facilitate coordinated care and referral.
- Liaises and participates in case consultations with the multidisciplinary team and family physicians to support collaborative patient care.
- Facilitates and develops psychoeducational workshops and groups for general and specific patient populations (eg. Managing Stress, Improving Self-Esteem).
- Participates in ongoing program evaluation through the diligent recording of clinical interactions and patient data in MPCN evaluation systems.
- Participates in ongoing quality improvement initiatives within MPCN and the Social Work Team.

**Qualifications & Requirements**

Education and Experience
- Bachelor of Social Work degree from an accredited university.
- Current registration with the Alberta College of Social Workers.
- Preferred minimum of 3 years’ experience working in a community-based agency with diverse adult populations.

Knowledge, Skills and Abilities
- Comprehensive knowledge of, and experience with resources related to basic needs, chronic disease, and social benefits programs at the community, provincial, and federal level.
- Demonstrated understanding of intercultural practice and experience working with diverse populations.
- Strong understanding of social determinants of health and demonstrated experience with addressing barriers to wellbeing in diverse and vulnerable populations.
- Demonstrated excellence in interpersonal, verbal, and supportive communication skills.
- Ability to work independently and as part of a collaborative multidisciplinary team, providing leadership on complex cases where socioeconomic and social barriers impact patient care.
- Strong time management skills and flexible working style; ability to adapt and thrive in an evolving health care environment.
