Founded in 1969, East York Meals on Wheels is a not-for-profit community service agency that delivers great tasting, balanced nutritious meals through the help of our dedicated volunteers. Our services offer much-needed social support for both East York/Leaside’s seniors’ community and residents with health challenges. East York Meals on Wheels is a registered charity funded by client fees, donations, memberships, bequests, grants, the Toronto Central Local Health Integration Network, and the City of Toronto. Our agency is a member of Imagine Canada, the Ontario Community Support Association, and the Volunteer Center of Toronto.

**Social Worker**: Position Type: (entry-level) Part-Time, 1 year contract

**Job Summary**: As a member of EYMOW’s Social Worker Team, you are responsible for conducting home visits to complete client assessments, completing intakes for all new clients, developing care plans, and conducting safety/risk assessments. Responsibilities also include overseeing the Telephone Reassurance program, providing education and outreach to potential clients and referral partners, and supporting colleagues by acting as backup to other EYMOW programs.

**Responsibilities**:

- Intake, assessment (via home or community visit), and general agency orientation for all new clients
- Creation and maintenance of client-centered care plans and regular follow-up as required.
- Perform home safety and client risk status assessments.
- Follow up on client requests for subsidy and conduct financial assessments to determine qualification.
- Liaise with the client’s circle of care and ensure the client’s medical team is informed of the request for meals and that any special diet needs are identified and documented.
- Preparing regular statistical reports for submission to the management team
- Along with the other team members, serve as back-up for Meals on Wheels Coordinator and assist in meal packing, delivery, and debriefing of returning volunteers.
- Assist with special events, fundraising, and the occasional Sunday or Statutory holiday coverage.
- Participate in the supervision of college/university students and volunteers.
- Other duties as assigned by the management team.

**Qualifications**:

- Successful completion of a Social Work degree from an accredited university (BSW+)
- Proof of registration with the Ontario College of Social Workers and Social Service Workers (OCSWSSW)
- Minimum of two years of experience working with seniors, people living with disabilities, or other marginalized populations in a health or social services setting preferred.
- Excellent organizational, clinical, communication, and interpersonal skills, with the ability to establish a good rapport with staff, clients, families, and referral sources in a multicultural setting.
- Knowledge of and experience delivering psychosocial/risk/financial assessments
- Self-directed, results-oriented, and capable of working independently and as part of a team.
- The ability to use critical thinking, organizational and conflict resolution.
- Proven ability to establish and maintain productive working relationships internally and externally with stakeholders and partner agencies.
- Demonstrated computer skills at an advanced level in using the Internet and Microsoft Office. Exposure to CIMS or AlayaCare databases is an asset.
- Demonstrates commitment to ongoing professional development and regular clinical supervision.

**Additional Information**:

- Interviews will be conducted in person.
- Hourly Rate: $24-26/HR
- Schedule: Monday, Tuesday, and Thursday between 9-5 pm with the occasional Sunday or holiday shift coverage
