**Position Summary**:
Responsible for day-to-day oversight of staff within the company’s Adult Day Programs, Seniors Active Living Centre and Home Support Services, to meet the evolving needs of clients to live well and to age in place. Responsible for planning, designing, implementing and overseeing services that are inclusive, that meet the wide-ranging social health and support needs of older adults and their care partners who are of diverse backgrounds in a variety of settings (home/community/SPLC sites).

**Responsibilities**:
**1. Provides leadership for teams with assigned programs and services***:

- Oversees programs and services to ensure client health and safety and the provision of client-centered, culturally appropriate services
- Establishes consistent standards and procedures and actively participates in program planning and evaluation
- Develops, plans and ensures implementation of all program and services for individuals and groups in collaboration with staff according to clients’ functional levels
- Works closely with staff to ensure care partners and clients' needs are met
- Ensures that the needs of the diverse multi-cultural communities are addressed and programs and services are delivered in as inclusive a manner as possible

**2. Accountable for administration of assigned programs and services, supporting and collaborating with responsible team members to**:

- Fulfill administrative requirements such as budget, funder reporting, and correspondence
- Compile, maintain and analyze in statistical information and records, client demand, staff availability, return on investment, scheduling, billing, attendance, equipment and materials inventory
- Identify and documents program issues and make quality improvement and/or corrective actions as appropriate
- Ensure team members are maintaining accurate records, i.e. care/service plan, progress notes and statistical reports

**3. Supervises and supports staff***:

- Leads by example, provides guidance, and support to ensure an inclusive work environment, monitoring and promoting work-life balance
- Establishes a growth plan for staff in alignment with growth of the programs ensuring succession plans are in place that contribute to individual, team and program development in the context of all SPLC programs and services
- Contributes to staff development by promoting and ensuring collaborative and supportive-learning and capacity-building
- Leads team meetings, shares strategies/approaches and encourages skill-building of team members
- Coaches staff to ensure programs are delivered in a safe and supportive environment

**4. Evaluates, redesigns and promotes programs internally and externally***:

- In close collaboration with the Director, Care Services, develops strategy and program design, planning and evaluation of assigned services based on community needs and resources available using a data-driven, evidence-based, decision-making approach
- Assist the Director, Care Services in developing both short and long-term operational plans in addition to funding requests
- Develops, plans and implements all services and programs in order to make optimum use of staff, facilities and resources and to ensure programs are meeting clients’ identified requirements, implemented through co-design wherever possible
- Promotes and markets assigned programs to ensure a broad awareness in the community
- Provides a wholistic approach to care where clients may be supported by multiple SPLC programs and services and collaborates with other teams as required

**Education**:

- Bachelor’s degree in a related field, or
- A regulated health professional registered with the corresponding Ontario college

**Experience and Skills**:

- Three (3) years’ experience working in with seniors; it will be considered an asset if this experience was within a health-care setting
- Two (2) years’ supervisory experience
- Ability to coordinate and work in an inter-professional setting
- Good understanding of gerontology issues, community-based care and the health care and social service systems
- Computer literate with advanced knowledge of Microsoft Office programs and use of social media and websites as part of communication strategies with clients and communities
- Fluency in a second language an asset such as Cantonese, Mandarin, or Tamil
- Experience working in a multicultural environment an asset
