**What you’ll do**

- Identify appropriate community services and participates in establishing relationships and connections to these services with patients. Assist in developing individual care plans with interdisciplinary team. Support information gathering to assist in appropriate triaging of referrals.
- Arrange, plan, escort, and accompany, as necessary through program clinical requirements, designated/marginalized individuals on medical appointments and errands, while providing emotional, moral support and assistance to further the enhancement or development of self-management skills.
- Create linkages to other community resources and programs, contacts relevant organizations/businesses/services, assists clients with processing forms. Support and advocate for clients when identifying and accessing social services, such as income security, food, supplements, housing, social networks, while supporting the development of self-efficacy and self-care.
- Act as a liaison between various agencies by methods such as updating designated team staff on services currently available and liaises with community agencies to provide information on services. Provide perinatal mental health and substance use information to community groups and health care providers regarding the sensitivity needed when working with vulnerable populations.
- With the Program Manager, coordinate in-reach supports from community partners. Act as main point
- of-contact for the scheduling of in-reach activities and brings forward recommendations for appropriate in-reach activities as part of the interdisciplinary team.
- Support research activities within the program as necessary. Task may include data collection and data imputation for a variety of research-based and/or quality review-based activities.
- Provide outreach support to patients referred to or involved in care at BC Women’s Mental Health & Substance Use Programs and Initiatives in a variety of settings including parks, encampments, shelters, supported housing and drop-in centers, for example.
- Provide clients with harm reduction support through individual risk reduction orientation, distribution of harm reduction supplies, and Naloxone distribution.
- Perform a variety of general clerical duties, attend meetings, and participate in patient care planning and coordination, as assigned.
- Perform other related duties as assigned.

**What you bring**

Qualifications
- Graduation from a recognized Social Services Worker certificate program or an equivalent combination of education, training, and experience.
- Valid Class 5 vehicle license.

- Ability to communicate effectively, both verbally and in writing.
- Ability to deal with others effectively.
- Ability to organize work.
- Ability to operate related equipment.
- Physical ability to carry out the duties of the position.
- Commitment to upholding the shared responsibility of creating lasting and meaningful reconciliation in Canada as per TRC (2015) and BC's Declaration on the Rights of Indigenous Peoples Act (2019).
