Overview:
Key Duties & Responsibilities:

- Conduct needs assessments and develop community integration plans for survivors.
- Develop, coordinate, and monitor goal-oriented rehabilitation plans to promote independence and development.
- Facilitate group programs focusing on skill development related to brain injury effects and management strategies.
- Provide education and support to survivors and families in both one-on-one and group settings.
- Offer information, referrals, and support in accessing various services within CBIS or the community.
- Assist with crisis intervention and ensuring access to essential community services.
- Provide or arrange transportation for survivor participation in rehabilitation programs and appointments.
- Organize and facilitate meetings with other service providers involved in the rehabilitation process.
- Establish and maintain collaborative relationships with health system professionals and relevant agencies.
- Write proposals for third-party funders and maintain documentation such as rehab plans and program reports.
- Evaluate program effectiveness through observation and client feedback, reporting findings to the supervisor.
- Coordinate and manage projects such as Men’s Shed, She Shed, Cognitive Digital Therapy, and others as needed.

**Skills**:

- Experience in social work, hospital or hospice care
- Background in social work, nursing or related field
- Knowledge of acute care practices
- Proficiency in case management
- Familiarity with pediatrics
- Strong understanding of medical terminology

**Job Types**: Full-time, Permanent

Pay: $25.00-$30.00 per hour

Expected hours: 35 - 40 per week

Flexible Language Requirement:

- French not required

Schedule:

- 8 hour shift
- Day shift
- Monday to Friday

**Education**:

- Secondary School (preferred)
