Established in 1978, SWCHC provides a wide range of health care services such as primary health care, health promotion, community development, mental health, and harm reduction. About ONHC

The Ottawa Newcomer Health Centre (ONHC) is a proud program of the Somerset West Community Health Centre that provides healthcare, offers timely, coordinated and culturally appropriate health and social services to newcomers. Our approach to care is through a health equity lens with a focus on addressing the social determinants of health to improve the lives and wellbeing of our clients.

Position Description

Position Requirements

**Education**:
Degree or diploma in social sciences or equivalent combination of training and experience
Degree or diploma in a health discipline or equivalent, an asset

**Professional Experience**:
Minimum of 2 years' experience in a community setting with emphasis on case management, community engagement, practical assistance, and case coordination support for clients
Minimum 2 to 3 years' experience working with diverse client populations, specifically with newcomers, refugees, and immigrants in Franco African and Black communities.

**Key Competencies**:
Minimum two years' experience in a community-based setting
Minimum three years' experience working with multicultural
Knowledge of the Canadian heath care system & social determinants of health
Basic health assessment and interventions, as appropriate
Sensitivity to and passionate about multicultural issues
Awareness related to cultural
Knowledge of related community resources
Ability to liaise with external parties and to advocate for clients and provide home visits as necessary.
Commitment to strength-based philosophy and practice
Assessment and intervention skills; sound judgment
Ability to provide basic education to individuals and families.
Excellent communication skills, able to bridge gap between clients and service providers/service agencies and advocate for clients.
Strong interpersonal and organizational skills
Ability to work independently and liaise with internal and external partners.
Must be able to work on multiple tasks simultaneously to meet
Respect the privacy of all clients and adhere to strict confidentiality protocols to protect client personal and sensitive health and social information.
Ability to work evenings and weekends, as required
Successful criminal reference check

Linguistic Profile (French, Swahili, Lingala)

Fluency in English, written and spoken - Mandatory
Fluency in French, written and spoken - Mandatory
Additional Language Capacity in Swahili or Lingala essential
