Detailed Overview: Reporting to the Manager, Mental Health & Substance Use Services, the Health Care Assistant provides routine care according to established standards and policies and in accordance with the Fraser Health's mission, vision and values. Working predominantly in an outreach role, in collaboration with the multi-disciplinary team, the Health Care Assistant performs duties including monitoring client's physical and psychological status during withdrawal and stabilization. In addition, the HCA assists with group/and or individual discussions to provide general program information to clients, their families and other community members, regarding lifestyle, substance use issues and maintaining optimum health. In addition, the Health Care Assistant performs admission and discharge duties and participates in client care planning in collaboration with the multi-disciplinary team. Using a strengths-based model, motivational interviewing and engagement techniques, the Health Care Assistant works collaboratively with the clients. Responsibilities:

- Using a strengths-based model, engages with clients, determines the stage of change, risk indicators and client needs.
- Assesses and monitors client's physical and psychological status during detoxification, withdrawal and stabilization by performing such duties as taking and recording vital signs including blood pressure, pulse and respiration. Records observations and reports problems and/or changes to designated nursing staff.
- Performs admissions, discharges, and referrals of clients according to program operating policies and procedures. Meets with clients upon admission to gather personal information including history and length of substance use and completes client documentation.
- Maintains and updates related records and documentation including medication records and client files.
- Contributes to the development and modification of the individualized plan of care, including attending and providing input into multidisciplinary care planning meetings and family conferences as required.
- Assists with discussion groups and exercise sessions. Provides education and information sessions for clients and their families on topics such as lifestyle, basic health, nutrition, substance use and behaviour management.
- Represents Withdrawal Management Services to ensure positive and productive community relationships by performing duties such as attending and participating in Health Fairs and Fraser East Addictions committee meetings to provide and share general information relating to wait lists processes, updates/changes to the Riverstone program, etc.
- Collaborates with multidisciplinary team and completes client discharges by meeting with clients prior to discharge, ensuring that treatment plans are in place prior to discharge.
- Assists in minimizing transportation barriers by transporting clients to WMS related appointments with physicians, to STLR/STAR beds, and in support of clients overall wellness plan as otherwise deemed necessary.
- Provides information regarding services provided and procedures to the clients, families and the general public.
- Performs other related duties as assigned.

Qualifications:
**Education and Experience**
Completion of Substance Abuse Counselling Certificate or Diploma program plus one year's recent, related experience or an equivalent combination of education, training and experience working with clients with problematic substance use or addictions issues.
