**Job Description**:
QUALITY RISK MANAGEMENT CO-ORDINATOR - Extendicare Medex

RESPONSIBILITIES
- Is the lead on the homes Continuous Quality Improvement team and assists as needed in the completion of the quality protocols and program evaluations. Creates action plans and ensures follow-up actions are completed. Facilitates quality projects through PSDA cycles, fishbone exercises etc.
- Lead / chair the quality improvement committee within the home
- Helps in development of the quality initiatives and support programs for residents with Behaviours, Palliative Care, Falls, education and other programs in the home
- Participates in conjunction with Programs on resident autonomous and quality of life initiatives.
- Oversee quality initiatives across the home with a focus on clinical quality indicators
- Design and implement quality improvement projects
- Develop solutions to existing quality improvement barriers
- Track and trend quality performance
- Attend and participate in local committee meetings
- Provide frontline presence on the units to support, evaluate and better understand quality and barriers to quality
- Use project management skills to lead teams towards common goals
- Identify gaps in training and knowledge and create programs to fill these gaps
- Manages other assigned projects, ensuring effective teamwork and communication, high quality of work, organizational performance and continuous learning.
- Assists in the collection of data for infection control
- Participates in Health and Safety. Audits committee, etc.

QUALIFICATIONS
- Registered Practical Nurse or other Regulated Health Professional, with Lean/Quality Improvement training is preferred
- Considerable experience in quality improvement and risk management
- Proven ability to analyze data, implement quality improvement, solve problems and resolve conflicts.
- Ability to collect and use information to thoroughly think through problems and situations; puts information from various sources together to see the ‘big picture’, and generate creative solutions
- Ability to work effectively individually and with others while prioritizing tasks to handle the most urgent first
- Organizational, planning, time management and multi-tasking skills
- Comprehensive knowledge of nursing and health care practices, as well as knowledge in infection control practices
- Experience in staff education in a healthcare or similar organization.
- Leadership style that creates learning, encourages participation and commitment to organizational objectives and values the contributions of staff.
- Excellent ability to motivate, lead, train and manage staff in a fast-paced environment.
- Highly developed interpersonal, conflict resolution, project management and problem solving skills.
- Ability to interact and communicate effectively both orally and in writing at all levels of the organization.
- Well developed management level report writing and presentation skills.
- Sound knowledge of government legislation related to long-term care and occupational health and safety.
- Knowledge of public policy, accreditation standards and processes that affect long-term care.
- Proven ability to meet deadlines and manage multiple assignments.
- Familiarity and understanding of electronic health record systems i.e. PCC, RAI/MDS, RUGS, E-MAR, POC tablets an asset
- Knowledge of LTC industry regulatory requirements and current Best Practices an asset
