Care Coordination Centre hubs across the Northern Zone supports teams to identify issues and find solutions together, using real-time data. This will translate into improved patient access and flow, efficient bed management and discharge planning, better scheduling of staff, and more collaboration across health zones across the province.

Reporting to the Director of Patient Access and Flow in Northern Zone, the Patient Flow Manager is a fully realigned and integrated role in C3 within the Northern Zone C3 hubs that incorporates the 24/7 patient flow and after hours administrative functions.
