- These hours are dependent on the operational necessities of the organization. Days/evenings and weekends may be required
- Mount Carmel Clinic operates out of multiple locations; you may be required to work in other areas
**Position Summary**
Functioning as part of a coordinated service delivery team, the Outreach Worker works proactively with new and existing clients, offering a range of support and recovery services, within an existing service plan and on an as-needed basis. The Outreach Worker is a relationship-builder and advocate and provides service in a non-judgmental and culturally appropriate manner, helping clients to build new skills and achieve their goals.
**Qualifications**
**Education**
- Must have a Valid Drivers’ License and access to a reliable vehicle.
**Service Delivery Skills, Expertise and Experience**
- Minimum of 2 years’ experience working in community, preferably with program target population
- Experience providing outreach with hard to reach populations
- Must have experience working with a vulnerable population
**Critical Skills**
Proven knowledge and ability in relationship-building, critical thinking, facilitation, decision-making, problem-solving and conflict-resolution skills
**Focus on Results**
Demonstrated ability to prioritize and focus work-related activities to effectively and efficiently achieve service goals and produce expected results/outcomes
