**Your Opportunity**:
Prehabilitation describes care, education, and resources provided before surgery designed to reduce the risk of surgical complications, reduce the need for unexpected healthcare services, and help patients return to normal activities more quickly after surgery. The Edmonton Zone Individualized Prehabilitation (EZIP) Social Worker III (SW III) is an exciting, new, and critical rehabilitation position within the Community Allied Health, Edmonton Zone. The EZIP team is a new interdisciplinary rehabilitation team that will be responsible for co-designing, creating, and delivery of Individualized Prehabilitation services in collaboration with the Provincial Prehabilitation Coordination Team. Prehabilitation interventions aim to improve or maintain health across five domains: physical, nutritional, psychological, social, and modifiable risk factors. The individualized services will meet the specific health needs of patients and families along with necessary referrals to the care providers through education and services in virtual, in-person, and group formats. The EZIP SW III will provide team coordination and leadership role along with a clinical caseload for managing clients who require counseling or education for managing social or psychological needs as they prepare for upcoming surgical intervention(s).

As a Social Worker III, you will perform either advanced level assessment and treatment or act as a clinical lead. You will work with or within teams to provide health care services for patients with complex and multidimensional medical, psychosocial, environmental and instrumental needs. You will provide advanced clinical services, consultation to the interprofessional team and patients/families and supervision of students and team members as assigned. You may also act as a clinical lead providing leadership in the planning, development, and delivery of services in a specific area, as well as staff development, quality improvement and evaluation or research activities as identified by the needs of the service area(s).

**Required Qualifications**:
Master's degree in Social Work. Equivalent education or experience may be considered. Active or eligible for registration with the Alberta College of Social Workers (ACSW).

**Additional Required Qualifications**:
Minimum of 8 years of experience in rehabilitation practice, including community rehabilitation and/or outpatient specialized service settings. Formal and/or informal demonstrated leadership experience with the ability to assess performance and provide ongoing development. Extensive knowledge of patient population needs, health care continuum, and leading practices in collaborative, interdisciplinary rehabilitation. Extensive knowledge of pre

Excellent clinical reasoning, decision-making, negotiation, and conflict resolution skills. Ability to foster relationships/communicate/network with highly complex internal & external stakeholders. The SW III may sit for long periods of time. In a community setting, there are occasionally some physical demands with transporting equipment, therapy materials (20-50lbs) or bending/rotation, etc. as required for clinical duties.

**Preferred Qualifications**:
Canadian Social Workers Association membership preferred or other organization closely affiliated to social work practice areas is an asset. BLS-HCP Certification required. Valid Driver’s License required with access to own vehicle that is insured as per Alberta Health Services policy and appropriate business insurance.
