**Full Time Outcome Support Facilitator**

**Summary of Responsibilities (including but not limited to)**:

- Ensure that the people you are supporting are always your top priority and provide the highest quality/standard of support expected
- Learn about, support and keep current on people’s Personal Outcomes, needs, preferences and requirements for the specific support they want
- Utilize Personal Outcome Measures as a guide to identify outcomes with people
- Participate in Personal Plans and be accountable to support people to follow through with appropriate planning, implementation and documentation required to attain results
- Support people to develop skills and assume roles, which promote participation in the life of the community
- Support people to develop decision making skills, exercise choices and assume responsibilities and reasonable risks
- Participate in the development of support networks and community partnershipsfor people and promote the involvement of family, friends and community members in people’s lives
- Identify and advocate for change of any barriers which inhibit or impede people’s Personal Outcome goals
- Ensure that all documentation, information, needed actions, and important observations, related to the support of people are passed on to appropriate team members, Supervisor and the Quality Enhancement Committee, where organizational supports are needed to aid in the implementation of change
- Liaise effectively and professionally with families, parents, community groups and employers
- Ensure an orderly and safe working environment and follow all policies and procedures
- Complete all necessary administrative and service delivery documentation as required by the organization on time
- Assist with daily financial audits for people using the AIMS database
- Order and purchase Health & Safety and emergency supplies
- Complete all duties related to H&S and serve as the Health & Safety Representative, if required
- Assist with petty cash report and monitor/identify discrepancies following process
- Contribute to monthly calendar setup of outcomes, supports and appointments for all people supported at location and report and/or problem solve discrepancies in vehicle use/transportation and supports needed
- Recognize that employees may at times need to use their personal vehicles to transport persons supported (and potentially one or more members of the supported person’s family) when vehicles owned by the Organization are not reasonably available, or on a regular basis when supporting in a program that may not have an Organizational vehicle
- Other duties as assigned

**Experience With (including but not limited to)**:

- Development and implementation of protocols, such as Behaviour Support Plans, Best Practices for Incident Prevention, Employee Safety Protocols, Safety Protocols and Health Protocols
- The use of and troubleshooting for, specialized equipment (i.e. walkers, lift systems, etc.)
- Experience with supporting complex medical conditions, such as seizure disorders, diabetes, anxiety and medication administration
- Controlled acts, such as suppositories, enemas, inhalers, etc.
- Extensive experience implementing NVCI techniques including both preventative and physical interventions and experience supporting people with challenging behaviours
- Supported Living, Life Share, SIL, Community Participation and Passport documentation requirements
- Supporting a person with Dementia and Alzheimer’s Disease
- Liaising with medical professionals, registered nurse practitioners, psychologists, psychiatrists, physiotherapists, pharmacists, behavioral consultants and communication specialists
- Multiple forms of communication for a person supported (i.e. sign language, communication boards, use of tablets and iPads, implementing communication plans and trials to gather data for communication consultants)

**Education and Qualifications (including but not limited to)**:

- Ability to remain flexible with support schedules to meet the needs of people
- Excellent attendance to ensure continuity and consistency for people supported
- Demonstrated ability to adhere to administrative expectations and timelines
- Proficiency with information technology including Kronos, AIMS and Surge
- Proven ability to maintain a professional attitude and handle stressful situations
- Exceptional interpersonal, oral and written communication abilities
- Strong organizational, time management and problem solving skills
- Able to maintain confidentiality and conduct work within appropriate privacy standards
- Demonstrated positive attitude, initiative and tea
