**Job Description**:
The Infection Prevention and Control Department (IPAC) is an integral part of Osler. As a member of this department, the IPAC Practitioner facilitates infection control activities and is considered a change agent with the primary goal of reducing risk of infections. The IPAC practitioner directly reports to the IPAC Hub Manager.

The IPAC Practitioner working with the regional IPAC hub will provide regional IPAC expert guidance for local implementation and sustainability planning in partnership with community congregate settings including LTC partners, retirement homes and other congregate settings (e.g., group homes, supportive housing and shelters).

Accountabilities:

- Providing clinical IPAC leadership to congregate living settings within the central west region
- Oversight and facilitation of local community of practice
- Flagging infection control concerns to setting and IPAC hub manager
- Supporting IPAC planning and capacity building
- Providing IPAC best practice and clinical direction
- Conducting clinical IPAC assessments
- Providing rapid access to IPAC expertise 24/7
- Working in collaboration with public health and Ontario Health supports
- Reporting on assessments and risks for identified homes and providing IPAC education appropriate to settings

**Qualifications**:

- A degree in health related discipline preferred (e.g. nursing, laboratory, epidemiology and other allied health)
- Current registration and in good standing with a regulatory body in Ontario
- Previous experience as an infection control practitioner in an acute care setting is preferred
- Completion of an entry level 1 infection control course required
- Certified in Infection Prevention and Control preferred or commitment within two years
- Knowledge and/or job related experience in health care practices
- Strong knowledge of infectious diseases, microbiology, epidemiology and statistics
- Proficiency in the use of computer databases, spreadsheets, Meditech and literature search
- Demonstrated ability to work collaboratively in a multidisciplinary team environment
- Self-directed, well-organized and detail-oriented
- Excellent interpersonal, communication and presentation skills

- Excellent attendance, punctuality and work record
- Demonstrated skill in teaching and strong written communication skills
- Able to participate in the IPAC on-call rotation and travel within the central west region
