**Program Overview - Crisis Outreach Service for Seniors and Toronto Seniors Helpline, Community Care**
This position works on behalf of WoodGreen, in coordination with partner agencies such as Home & Community Care Support Services, Haven Toronto, St. Michaels Homes, South Riverdale Community Health Centre, Reconnect Community Health Services and LOFT Community Services to provide seamless crisis and service navigation support to seniors and their caregivers 365 days/year.

**What You Will Do**
SPECIFIC ACCOUNTABILITIES - SCHEDULING SUPPORT
- Ensure the scheduling cycle and staffing workflows meet the needs of the crisis programs and are accurately documented in appropriate systems
- Ensure service is accurately scheduled for Crisis & Resource Counsellors, Crisis Specialists, Crisis Workers (including full-time and relief staff) for a program in operation 7 days/week (Mon to Fri 9:00am - 8:00pm and weekends and statutory holidays from 9:00 - 6:00pm).
- Prepare scheduling assignments for teams 3 months in advance according to availability and skill set.
- Organize coverage for unexpected absences/employees who call in sick by calling relief staff
- Approve schedule changes (e.g. shift switches, vacation requests, sick time and training requests) in accordance with collective agreements, ensuring adequate staffing levels
- Communicate vacation/shift change approvals or denials to staff and managers based on unit criteria and collective agreement.

SPECIFIC ACCOUNTABILITIES - OPERATIONAL AND CLINICAL SUPPORT
- Offer clinical supervision and consultation support to frontline staff.
- Support senior manager with recruitment, interviewing and retention of staff.
- Provide/organize orientation and training for new COSS and Toronto Seniors Helpline (TSH) staff
- Provide/organize supervision for COSS and Toronto Seniors Helpline students.
- Provide after-hours clinical/scheduling support to TSH and COSS staff including weekday evenings and weekends.
- Provide clinical consultation, support and regular contact with relief staff.
- Monitor staff performance targets and data quality by regularly reviewing documentation in client management systems, calls reports, statistics and consultation with staff.
- In collaboration with senior manager, supervise and manage any performance issues.
- Provide support to COSS teams for internal transfer of clients.
- Support staff with approvals for personal support workers.

SPECIFIC ACCOUNTABILITIES - SITUATION TABLES (FOCUS and SPIDER)
- Represent COSS at FOCUS 51 and SPIDER tables - sitting at weekly/biweekly meetings and other collaborative tables on operational needs & growth
- Follow up on all COSS referrals to FOCUS and SPIDER and collaborate with staff and management on COSS West (Reconnect) and North (LOFT) teams
- Maintain timely response to support requests and ensure timely follow up with partners and referral sources

SPECIFIC ACCOUNTABILITIES - PROGRAM DEVELOPMENT
- Support expansion of TSH and COSS to a 24/7 model
- Conduct outreach for crisis programs to maintain awareness in the community.
- Maintain COSS SharePoint site with relevant resources and tools for the team.

ACCOUNTABILITIES - GENERAL
- Work effectively with members of the unit and other program area staff.
- Work effectively with partner agencies to develop collaborative approaches
- Maintain responsive communication via various channels with all crisis team and WoodGreen staff and partner agencies
- Support senior manager with data quality objectives
- Ensure the equity and anti-racism objectives are achieved within the program.
- Participate in regular supervisory sessions with the senior manager.
- Upgrade training and education as directed by WoodGreen to fulfill job responsibilities.
- Maintain familiarity with all of WoodGreen’s policies and procedures.
- Perform other related duties as required from time to time.

**What You Bring to the Team**
- Master’s degree in in Social Work or comparable education in a relevant field.
- Registration with the Ontario College of Social Work and Social Service Workers, or equivalent.
- 5+ years of clinical experience in mental health and/or addictions and seniors’ sector
- 2+ years progressive leadership experience in the social service/ healthcare sector. Clinical skills related to supervision of staff/students, assessment of client needs, partnership-building and professional, timely communication with staff from multiple partner organizations.
