**Purpose of Job**

Island Sexual Health Society (ISHS) is a community-governed, not-for-profit organization dedicated to providing low-barrier, culturally safe primary and sexual health care. The Social Worker is a member of the Allied Health team and plays a vital role in providing patient-centered psycho-social care to a diverse range of clients.

The range of specific responsibilities will include but not be limited to the following:_

**Patient-Centered Care Coordination**:

- Collaborates with the Allied Health team to develop individualized support plans that reflect each patient’s mental health and social needs
- Attends regular case management meetings with the Clinical Nurse Lead to provide input on care plans, ensures continuity of care, and addresses emerging patient needs
- Completes biopsychosocial assessments to collaboratively identify needs and goals, with input from the patient and interdisciplinary team members, then documents the biopsychosocial plan and acts as point on the plan for team members

**Patient Counseling and Support**:

- Provides empathetic, in-scope, short-term individual and group counseling to address immediate emotional, psychological, or social needs
- Determines counselling timelines and care plan
- Guides patients through difficult transitions and offers compassionate support tailored to individual backgrounds and identities

**Crisis Intervention**:

- Delivers crisis support for patients experiencing mental or emotional distress, utilizing de-escalation techniques to prioritize patient and team safety

**Community Program Facilitation**:

- Designs and leads group programs, such as art therapy or substance use support, fostering an integrative approach that considers patients’ physical, mental, and social health

**System Navigation, Referrals, Resource Networking & Documentation Assistance**
- Facilitates referrals for patients requiring long-term clinical counseling, substance use support, and detox services
- As case manager, completes all system navigation tasks with the patient and acts as a liaison to ensure smooth transitions and continuity of care
- Connects patients with external support networks and community resources
- Completes necessary forms and documentation to assist patients in navigating healthcare and social service systems effectively
- Creates and maintains a collection of printed and digital resources for patients

**Additional Responsibilities**:

- Provides clinic team mental health and wellness supports and programming (pending case load)
- Oversees other mental health-related projects for the clinic (pending case load)
- Adheres to infection control guidelines and protocols, including regular hand hygiene, use of PPE, and proper disposal of biohazardous waste
- Participates in ongoing training and education programs on infection prevention and control practices to ensure the highest level of safety and protection
- Additional responsibilities as assigned

**Education, Licenses and/or Certification, Experience Required**
- Bachelor of Social Work (BSW) or Masters of Social Work (MSW) from an accredited institution and registration with the British Columbia College of Social Workers (BCCSW)
- Minimum of two years working with marginalized populations within a clinical setting
- Certification and/or advanced training in evidence-based counselling and crisis response models including cognitive behavioural therapy (CBT), dialectical behaviour therapy (DBT), and suicide assessment and risk mitigation

- Ability to establish and maintain healthy professional boundaries with patients
- Skilled in working independently and within an aligned interdisciplinary team
