You will play a critical role in supporting the emotional, social, and psychological well-being of our residents. You will work closely with residents, their families, and the care team to ensure that residents receive the best possible care and support.

**Key Responsibilities**:

- **Assessment and Planning**:

- Conduct comprehensive assessments of residents' social, emotional, and psychological needs.
- Develop and implement individualized care plans in collaboration with residents, families, and the care team.
- Monitor and evaluate residents' progress and adjust care plans as necessary.
- **Counseling and Support**:

- Provide counseling and emotional support to residents and their families.
- Facilitate support groups and educational sessions for residents and families.
- Assist residents in coping with the challenges of aging, illness, and loss.
- **Advocacy and Coordination**:

- Advocate for residents' rights and needs within the facility and the broader community.
- Coordinate with healthcare providers, community agencies, and other resources to ensure comprehensive care.
- Assist residents and families with navigating healthcare systems and accessing available resources.
- **Documentation and Reporting**:

- Maintain accurate and up-to-date records of assessments, care plans, and interactions with residents and families.
- Prepare reports and documentation required for regulatory compliance and quality assurance.
- **Team Collaboration**:

- Collaborate with the interdisciplinary care team to ensure holistic care for residents.
- Participate in team meetings, case conferences, and care planning sessions.

**Qualifications**:

- **Education**:

- Bachelor’s or Master’s degree in Social Work from an accredited institution.
- Current registration with the British Columbia College of Social Workers (BCCSW).
- **Experience**:

- (preferred) 2 years of experience in social work, preferably in a long-term care or healthcare setting.
- Experience working with elderly populations and individuals with chronic illnesses or disabilities.
- **Skills**:

- Strong interpersonal and communication skills.
- Ability to work effectively in a multidisciplinary team environment.
- Knowledge of community resources and healthcare systems.
- Empathy, patience, and a strong commitment to improving residents' quality of life.

**Job Types**: Full-time, Fixed term contract
Contract length: 6 months

Pay: $54.40-$57.80 per hour
